This patient was referred to see me after having several try-ins at other dental offices and not being satisfied with the aesthetics. He is very particular and aware of the details that make the teeth look natural and not fake. He also has one tooth that’s severely discoloured (notice the difference in translucency on the left central incisor). First thing I did was have a conversation to understand what he liked and didn’t like about the crowns he had tried in. After establ...ishing that one of his concerns was the appearance of the teeth to the gums, I refined the preps and margins to be 0.5-1mm subgingival to allow for more ideal emergence profiles and contours. I then completed a virtual smile design and communicated all the details to the lab via photos and the smile design images. I really enjoy these challenging aesthetic cases where the details make all the difference. What’s your approach for cases like this?

This amazing young lady came to see me with a failing lower incisor implant. Can you guess why it failed? It’s difficult to know for certain but I would be confident saying it failed due to not enough restorative planning and attention to occlusion. The implant crown was guiding all forces in protrusion and there’s been significant wear on the opposing upper incisor (with no signs of any significant wear on the adjacent teeth). The implant was removed and the area grafted.... After discussing the options we opted to restore the missing tooth with a conservative one-wing resin bonded bridge. Almost no preparation to the adjacent tooth needed. The cantilevered lithium disilicate pontic is completely out of occlusion. Sometimes simpler is better!She will do very well with this long-term and I expect the soft tissue to heal nicely and partially fill the interdental areas in the next several months.

Not all smile makeovers are created equal... This patient came to see me a couple years after having a smile makeover done elsewhere. Ever since having those veneers placed (upper photo) -- she had experiencing pain, bad smells in her mouth, and swollen gums. She was also unhappy with how fake her teeth looked. I removed most of her upper and lower poorly fitting veneers -- treated the teeth and the gums to get them healthy again -- and made her a new natural looking smile (lower photo). #qualityoverquantity

Virtual smile design is something I use regularly with my patients to show them what can be done to transform their smile -- and get their feedback to better understand their concerns and expectations. It certainly beats the alternative of having to rely on just verbal descriptions. It doesn't end there though. I also use these virtual smile designs to more precisely communicate with my lab. They're able to take this 2D smile design --- and replicate it in the 3D design for the virtual wax-up. #predictability #communication

I often get asked about what the best digital scanning protocol is for crown preparations. Here's a video that shows my go-to sequence for scanning. This video is sped-up but you can see the actual time taken for each scan in the upper right corner. Digital impressions significantly save time, and allow for instant feedback regarding the prep designs, margins, occlusal clearance, etc. High resolution "zoom" scans act like light body PVS in capturing the details of the preps ...and margins. The locking surface tool allows me to lock the position of the retracted gingival margins -- so that any further scans do not change those critical areas that had been retracted to show the margins. High definition photos are very useful when they're overlaid to the scan when it comes time to mark margins and identify where the prep stops and the soft tissue starts. See more

Here's another example of a resin bonded bridge to replace the missing lateral incisors. Because of the occlusion, no preparation was needed on the canine (except for slight reshaping to get more mesio-distal space). This was a zirconia single-wing RBB -- which was a great solution for this young man to get his smile back within a couple weeks. Check out Dr. Markus Blatz's APC zirconia bonding concept -- which is what I used to bond this restoration in place.

I'm a big fan of single-wing resin bonded bridges (RBB). In the right cases, they're the perfect solution. Very minimal prep (if any) is needed on one tooth, and there's plenty of evidence that shows that with proper planning and bonding, we can achieve outstanding long term results.

Transformation complete! This wonderful lady had old dentures she never wore (she hated them). We were able to use dental implants to give her completely fixed upper and lower teeth that she can now eat and chew with. I’m pretty sure she now smiles a lot more too!
