Why high Insulin blood levels may not be a good indication of what is happening in the foot.. Laminitis can be caused by many varying factors, however scientist...s have recently been concentrating on the pathways of inflammation in laminitis caused by high insulin levels. High insulin levels do not necessarily indicate insulin resistance, insulin resistance is hard to diagnose and is most often linked to horses with PPID. This latest study chose to use three groups of horses and to raise insulin levels by administering insulin /dextrose until Obel grade 2 lameness was achieved to one group. To another group of horses only dextrose was infused to moderately raise insulin levels similar to eating a high glucose starch /meal and the third control group were given an electrolyte solution for the same period. The study then looked at inflammatory markers and in particular studied Toll Like Receptor 4 which has a well known history of being the start point or the first signal of many disease states,human medicine has defined no less than 10 different TLRs that will respond at the onset of a disease state. This new study looked at TLR 4 which has a well documented history of being involved in defense against a wide range of gram negative bacteria in particular Pseudomonas aeruginosa which causes eye problems in horses, and Ecoli which causes gastrointestinal problems. TLR4 is activated by Heat Shock Protein 60, Heat shock proteins are responsible for maintaining cellular integrity in response to environmental changes. Stresses such as temperature, concentration imbalance, pH change, and toxins can all induce heat shock proteins to maintain the conformation of the cells proteins. Human and animal studies have linked HSP 60 to diabetes, stress response and cancers. TLR 4 also responds to certain respiratory viruses The aim of this study of laminitis was to see if TLR4 was increased in the lamellae and skeletal muscle when insulin levels were high.The study began by comparing levels of TLR4 in normal horses, from lamellar tissue and skeletal muscle in the healthy, control horses. TLR4 protein was present in both tissues, with no difference in protein expression between these two tissue sites. The next step was to discover whether high insulin levels could activate TLR4 and subsequently increase IL-6 and TNF- expression in insulin-sensitive tissue. The study showed that there was no increase from the skeletal muscle. The third step was to see if TLR 4 increased in the lamellae tissue and there was indeed a raise in the crude membrane protein fraction compared to controls, which was consistent with increased activation of TLR4 signalling. Lamellar protein content of the pro-inflammatory cytokine TNF- was also increased compared to paired control horses, without a change in IL-6 protein content. Compared to control horses, there was a trend towards an increase in lamellar SOCS3 protein content following the infusion of insulin whilst the horses given dextrose did not show an increase in TLR4, IL-6, TNF- and SOCS-3 when compared to the control horses. This would indicate that by high levels of insulin in the blood may not be an accurate indication of what is happening in the foot and it also indicates that eating a moderately high starch meal may not cause the onset of inflammation in the foot, however any condition that increases circulating free fatty acids such as obesity is likely to cause an increase in the expression of TLR4 and the onset of a disease such as laminitis. Today Pinecrest Stables is Waverly, ON was host to an Applied Equine Podiatry Clinic. Thanks to everyone who took part! There was a lot of learning and myth-busting going around! Topics included the truth about Navicular, the importance and real job of the frog, the dangers of the most common "fixes" for hoof issues, the anatomy of the equine foot, and much more!
