Nope...It’s Not a Zipper! As a pelvic physiotherapist, I meet MANY women who are concerned about their bellies after giving birth. For most, it is the appearance of their abdomen that is the focus of their attention and for a greater number, they want nothing more than for me to help them close the gap! Being a woman and a mother of twoI understand the esthetics of the affectionately termed mommy tummy (hint: a woman will never feel affection from this label being ...used to describe their abdomen). And for many years, there was an eternal quest to zip the zipper. Well ladies, in the words of Diane Lee (Clinical Specialist in Women’s Health), there has been a world-wide paradigm shift in how we treat women with diastasis recti. And closing the gap should not be the aim of therapy. Diastasis recti (DR) is the stretching of the midline found in between the abdominal muscles. It can result from pregnancy or improper use and loading of the abdominal muscles over time. Research has shown that there are no clear risk factors for DR and there are no tricks that can be done during pregnancy to prevent a DR from occurring. The abdominal wall is an incredibly important part of our body’s container and poor function may result in low back pain, pelvic pain, pelvic organ prolapse, and incontinence. Just as a diastasis can look differently on different women, the treatment may also differ. To say that this has been a year like no other would be a true understatement! Not only did ABIM grow and weather the storm... like all of you...we weathered it COVID style!! And we did so together...an amazing team that I could not possibly be more proud of! When the stakes were down, we pulled together and we gave it all we had! On the hardest days, we danced it out! We kept each other and all of our wonderful patients positive, motivated and believing that together...we co...uld move mountains! With the holiday season upon us, I am feeling incredibly grateful despite some bumps in the road...ok, potholes...really big potholes!! I am grateful for a team that could see the rainbow despite the storm! This year I was unable to celebrate with my amazing work fam so I came up with some fun ideas to let them know what they mean to me!! It seems that they had the same idea!! Team ABIM made a donation in my name to an organization that assists with prolapse surgeries and provides reproductive education and personal hygiene products to women that would otherwise go without! There is truly no better way to make my pelvically passionate heart happy! And...to commemorate our COVID year, they put together a video that was just too funny not to share!! So thankful! How’s That Meniscus? One of the most common knee injuries that we see in clinic are torn meniscus injuries. The agility that athletes have on the court, the field, or the ice, can be one of their greatest talents. The ability to swerve, reverse, sprint, jump, and dunk is their strategy to confuse their opponents however, one wrong move could cause significant damage. Although athletes in contact sports are more at risk for this type of injury, anyone can present with a torn ...meniscus regardless of age or activity. Wear and tear can result in meniscal injuries as well. The knee joint is a well made joint in the human body. It is typically a tough and strong joint that can withstand a significant amount of force. The meniscus is the knee’s shock absorber and is a C shaped cartilage that rests in the surface of the tibial plateau and houses the femur. There are 2 meniscus wedges in each knee. Damage to the meniscus will commonly result in: A pop at the time of injury. Pain in the knee joint may be persistent or may come and go and may increase in intensity when pressure is put on the joint. Feelings of swelling, fullness and/or stiffness. A feeling that the knee locking, clicking, or giving way. Untreated meniscal tears may result in the torn portion of the cartilage coming loose and floating into the joint, an increase in the size of the tear or further complications which may require surgical intervention to restore the function of the knee. Physiotherapy treatment early on can help alleviate your pain, restore your range of motion, as well as your strength, your stability, and your control. Your rehab program will focus on the knee itself as well as the muscles, tendons and ligaments that surround the injured area and will incorporate stretching, stretching, gait training, balance and agility training, and sport specific training. Nineteen years ago, today, I took my passion to create a unique and fabulous healing environment, and I started on a journey. Nineteen years later, I could not be more grateful!! My incredible work family spans the decades. Some have come, some have moved on and some have stay...ed for a long time. Every member of my work family has brought something special to ABIM and all have contributed, in some way, to making ABIM fabulous. The lifelong friendships will always mean the world to me! Thank you Team ABIM! Many of you as patients, have long been a part of ABIM while others have been with us for a shorter time. And I cherish every opportunity to welcome you! Do You Need to Get Back on Your Feet? You awake after a great nights sleep. You yawn, you stretch and you swing your legs to the side of the bed and sit up. You are ready to take on the day and you take your first steps and you are paralyzed with a sharp, stabbing pain in your heel. Sound familiar? This my friends is plantar fasciitus! Those are big words so let’s say them togetherPLAN-ter fash-ee-EYE-tus! And now, you will hobble through your day, perhaps take some painkill...ers to help get you through, and wonder what you can do to make it all go away! I think we can all agree that this is not a good way to live! So what is plantar fasciitus? There is a very tough band of tissue that runs the bottom of your feet from your heel bone to your toes. Plantar fasciitus occurs when this tissue becomes irritated and/or torn. A stabbing pain in the heel is the most obvious symptomone that is painfully obvious when you take those first steps in the morning. Pain my be felt in the heel, the arch or the ball of the foot and may begin as a dull ache that worsens over time. Plantar fasciitus can occur in both males and females, athletes and non-athletes and it can result in knee pain, hip pain or back pain because in order to compensate for the pain, people often change the way they walk. What can you do about plantar fasciitus? Typically, the earlier you seek treatment, the quicker you will be back on your feetso to speak. Our physiotherapists will do a full assessment to determine the most likely cause(s) of your foot pain. We will look at your posture, how you walk, and the make up of the muscles, tendons and ligaments in your lower extremities. We want to help you healand keep it that wayso determining the root cause is crucial. Treatment may include manual therapy to help to elongate the fascia, pain control techniques, the use of modalities, an assessment of your footwear, the development of an exercise program which will include both stretching and strengthening. Your exercise program will be developed so that you can perform it both in the clinic and at home. That way you can continue to heal in between your clinic visits! And most importantly, we will educate you on how to avoid a re-occurrence! So come and heal with us! September 13th to the 19th is Balance Awareness Week and if you are visiting our Boardwalk location for physiotherapy or massage therapy services, you will notice pink flamingos around the clinic. The flamingo is the mascot for balance and vestibular function. I have a large task ahead of me as I start to shed light and raise awareness around balance deficits and what the word vestibular means. But it’s worth it why? Because 1 in 5 people suffer from vestibular d...ysfunction. This means that 1 in 5 people have difficulty with balance, trouble reading, are unable to drive, experience frequent vertigo, require time off work and assistance with normal daily tasks, require a walker or cane to walk safely, are uncomfortable being left alone or are reliant on medication to suppress their symptoms. Even though there are no outward signs of injury or illness, this can be extremely debilitating! With the correct assessment and treatment, recovery is possible! That’s why I am so strongly advocating for the awareness and understanding of balance and vestibular dysfunction. Try this: pick a point in the room to look at. Now without moving your eyes from that point, turn to your head to the left about an inch and then to the right about an inch. When your eyes stay focused on that point during your head movement this requires the vestibular system to coordinate a reflex that allows your eyes to fixate while your head moves. This is an important reflex. This is an important topic. If you have trouble with balance, coordinating your arms and legs, difficulty reading or difficulty with mathematics and spatial awareness, frequent vertigo (the room spinning around you) or unsteadiness and unexplained falls, contact us today to set up a vestibular physiotherapy assessment. It is The Most Wonderful Time of the Year!! Although back to school is looking anything but normal this year, we do want to ensure that everyone stays safe! As physiotherapists, we see backpacks contributing to back pain, neck pain, should pain, and headaches! When we combine the posture that comes with carrying a backpack that is too heavy or not fitted properly with the continuous posture that comes from looking down at their phones, we have a recipe for postural dysfunc...tion. So, how best to choose a safe backpack for the school year? START WITH THE BAG: choose a lightweight bag (canvas or vinyl are best) with a padded back, two wide (around 2 inches in width) shoulder straps, a hip or waist strap and lots of pockets. MAXIMIZE THOSE POCKETS: pick a pack with lots of compartments, and use them, to help balance the load. HEAVIER ITEMS IN THE BACK: pack heaviest items closest to your back. Make sure items in the backpack are not sliding around. PACK IT LIGHT: The backpack should only contain what is needed for the day. Kids in grades K-8 should not carry more than 10% of their total body weight. Kids in grades 8 and up can get away with up to 15%. Weight adds up quick so make sure to weigh that pack. WEAR IT RIGHT: Place the backpack on a flat, waist-level surface and slip straps on one at a time. Adjust the straps so the pack sits flush against the back, it should be fit well but not be too tight (if you cannot slide a hand between the pack and your child’s back it is too tight. Use the hip or waist strap, it reduces the strain on the back. A good fitting backpack will do the job and keep our students healthy! Exercise Really Is The Best Medicine! You have probably heard it said that exercise will make you feel better. And maybe you’ve experienced this yourself. But let’s explore WHY it makes you feel better. Not only does research show that regular exercise, both endurance (such as walking and biking) and strengthening (such as using resistance bands and/or weight training), helps to decrease high blood pressure, manage and/or prevent diabetes and reduce the onset of arthritis ...or manage arthritic-related symptoms but it also demonstrates positive benefits of exercise on mood and levels of anxiety. Regular exercise causes the release of feel-good chemicals endorphin and serotonin, as well as other naturally created substances that increase your sense of well-being and happiness. This is why exercise can be so potent for those who are experiencing sadness or depression-like symptoms. Exercise also increases blood flow to the brain and this can help you to think more clearly and support over-active parts of the brain with nutrients and remove metabolic by-products. Additionally, exercise increases the size of the hippocampus. This is the part of the brain that is responsible for memory! That doesn’t mean you will have a photogenic memory but it might help you remember why you went into that room! Beyond the biological benefits, exercise allows you to gain confidence and boost your self-esteem by accomplishing physical tasks and reaching goals. It can also increase your social interaction with others. Exercise classes are a great way to be reminded that you aren’t in this alone and there are others who are on the same path to wellness as you; you might even make some new friends! Lastly, because of the biological effects of exercises, it is a positive way to cope when facing stressors, depression and anxiety. There are lots of methods of coping but some of them will only exacerbate the symptoms or the issues. Exercise provides a positive outlet for coping with conflict and negative moods. Next time you are faced with a challenging situation or feeling a little blue, try going for a walk. Physiotherapists are exercise-based health care providers. Not Just a Tennis Injury!! Lateral epicondylitis or tennis elbow presents as pain on the outside aspect of the elbow and occurs because of the overuse of the tendons in the forearm. These tendons are repetitively overloaded and eventually the result is pain in the elbow, the forearm, and/or the wrist. It is not just athletes that suffer with this painful condition. Any form of repetitive motion and stress on the tendons may result in tiny tears to the tendons that attach to y...our forearm muscles. Many people perform the same task consistently throughout their workday or with participation in sport and this may result in overuse of their tendons. Along with sport and occupation, age is a risk factor. Although tennis elbow can affect anyone, it is commonly seen in adults between the ages of 30 and 50 years (mayoclinic.org). Some common signs that you may have tennis elbow include pain with gripping an object, shaking hands, turning a doorknob, or holding your coffee cup! And a tell-tale sign is the application of pressure to the lateral epicondyle or the outside elbow bone. This area may become swollen and direct pressure will cause an immediate pain response. Inflammation is your body’s way of beginning to help the tissue repair. Our physiotherapists have experience treating both acute tennis elbow as well as persistent conditions that have failed to heal over time and your treatment will always be tailored to you and your injury. Acute injuries will require the reduction of stress to the tissues and assistance with the active healing process that your body will have begun. Chronic injuries will suggest that your physiotherapist re-start the healing process in a controlled and responsible manner and will include manual therapy, stretching, strengthening and the task of reintroducing you back into your occupation or sport with the goal of keeping you safe. Let's Talk Stress Fractures!! Have you ever heard the term or been diagnosed with a stress fracture? It is a relatively common injury caused by repeat loading/movement on the same bone over time. For example, running long distances increases the load or pressure put on the bones in the legs in a repetitive manner. This can cause stress fractures. How are stress fractures developing you ask? In a healthy bone, the cells are actively replacing old cells with new stro...ng cells. If there is too much loading on a bone too quickly and the bone doesn’t have time to prepare, then some of the older cells can break and form a microscopic crack. If this high repetition activity continues, the crack can get larger. Similar to a crack in your car windshield! Because of the nature of stress fractures, they are more common in the legs the weight-bearing limbs. They are commonly reported in the shin bone and the little bones of the feet. Factors that can put you at risk for stress fractures include being female, a low bone density, history of stress fractures and low food intake (poor nutrient status). Treatment will start with education of the issue/mechanism that caused the injury and then a crucial period of rest from this activity. Rest may include using crutches or wearing a weight-bearing foot (Air Cast), depending on the site of injury. Slowly, as the fracture site heals, progressions will include low intensity (non-weight bearing) exercises to maintain muscle and mobility, and then when there has been a consistent report of pain-free rest, physiotherapy works to rehabilitate the area and strengthen muscles to prevent or reduce the chances of re-injury. During this time, physiotherapy will also assess your risk factors and teach you how to modify tasks or the environment. So next time you hear the phrase stress fracture, don’t panic!
