We are proud to launch the expansion of My Wellness Passport, YWHO’s measurement-based care platform. 10 sites across Ontario will be implementing this platform... within mental health and wellness services for youth aged 12 to 25 years. The 10 new sites are known as My Wellness Passport sites. These sites were selected following a call for proposals that was shared across various networks in Ontario in June 2021. A huge thank you to our funders, Bell-Graham Boeckh Foundation Partnership (Bell Let's Talk), Balsam Foundation and Medavie, who’ve made this possible.
