Please be sure to fill this form out EVERY time you check into the gym. As always you MUST scan your key fob upon entry, this is your registration for time, as well tracking contact list. Masks as always are a must at ALL TIMES in the club except when physically exerting yourself. A distance of NINE FEET is now required for anyone not of your household. Team Training Sessions will be limited to 5 people now.
