Education is one of the most important parts of an initial consult as it will set the stage for any future treatment and the expectations that come with it. ... Nathan outlined 4 common and simple patient questions that can be answered right off the hop: 1. What is the specific problem? 2. How long is it going to take to get better? 3. What can you (the clinician) do to help? 4. What can I (the patient) do to help? For runners: 1. Is it a common injury? 2. Can they continue running? If not, what's the path back to 100%? P L A N T A R F A S C I A is one of the most common contributors to heel pain. A recent podcast I listened to featured one of the author's of a recent study that spent 5 years developing a clinical practice guideline for the management of plantar fasciopathy/(itis) by synthesizing data from systematic reviews, expert opinion, and patient values. ... I thought it was a unique approach and their findings are the reason for this post! 1st line treatment: stretching, taping, and individualized education based on load management, pain, footwear, etc. 2nd line treatment: shockwave therapy, custom foot orthoses (> prefab) Need more data treatments: exercise/loading, IMS, injections (cortisone, PRP) I was most surprised not to see plantar loading in the 1st or 2nd line of treatment but the author cited a lack of high quality evidence as the reason for it's omission. Clearly clinicians aren't finding success with it either in large and not surprisingly patient's probably don't love it/ find it valuable because it's generally uncomfortable. I'm confident loading will ultimately find its way into the treatment plan but perhaps at different stages of healing or with different parameters than are currently used. Often I will get a "nothing " which can lead to some confusion as to what happened and why they're now in pain. ... I think a great potential explanation to this question was proposed during the @runsmarterseries podcast with Ian Griffiths where they took the capacity-cup analogy and flipped it in reverse. Rather than an increase in load - cup overflowing scenario, the load has stayed the same while the cup got smaller, thus creating a relative overload from the same stimulus that at one point was tolerated well. Rather than always exploring the "what happened?" route perhaps we should also be asking "what didn't happen?" Did the coffee cup become an espresso cup? Looking back, a significant change for many people during the pandemic was the loss in ability to lift weights in the gym at the level they were before. The idea that performance and health are always positively correlated is a concept that doesn’t hold true in many a case. Rebecca posed this question at the end of her discussion on RED-S, which can be an extreme dichotomy between health and performance. As you push to train and perform at a given level, your body is routing all its efforts to give you energy at the expense of many internal systems functioning properly. Without the proper nutrition to sustain the training a... energy deficit grows. It’s possible to stay in this state for some time before injury, illness or something else catches up derailing your training and subsequently performance. This concept ties in nicely to a concept Phil Maffetone talked about... fit, but unhealthy ... and a parallel idea of the positive correlation b/w fitness and health. His discussion was primarily wrapped around his experience with endurance athletes. Some of whom are constantly in a state of sickness and over-training, which is sometimes worn as some kind of badge. Health has so many more facets than just fitness or performance and if you want to have sustainable fitness/performance at a high level or improve on it, Then nutrition, sleep, recovery, mental well-being and training all need to be in balance. If you have a significant change in training workload all the other aspects need to change, as well. My own personal anecdote rings true. In March of 2020 I was off work for a period of time due to the pandemic. I was bored so decided that I would try and run 30 straight days. I was not overly strategic with my running and had a high RPE with almost every run. At the end of the month I set a wild PR at 21k but my body hated everything about it. Sure my performance improved but nothing about what I did was healthy and it certainly was not sustainable without major changes to my nutrition and recovery process. Some food for thought.

Whoop had a new podcast on how the use of their recovery score showed an improvement in performance and reduced injury. If true, that’s huge. The details of the study (which was done by Whoop and haven’t been released ) are described on the podcast and detailed in my blog. The methods sound good but I’m always sceptical and there’s an obvious source of bias... With that being said, over 8 weeks, both test groups (whoop vs control) showed similar improvements in 5k time but ...the whoop group did it with less training time and a reduction in injuries! But I digress, if you’re interested check out the podcast. It was actually the guest host of the podcast, Mary Cain, who interested me the most! Perhaps you’ve already heard her story, but it was new to me. It highlights the importance of respecting the physiological differences between men and women with regards to training and culture. She outlines her story in a NYT opinion piece. A must watch/read. (Only 9700 followers away from the link attachment ) google it! The guys over at the Runners Zone have 2 super relevant podcasts with some expert guests on the respective topics RED-S and pelvic health. What this really underscores is the importance of having the right connections to experts in your area of practice. No one person is going to be able to tackle all of these issues on their own. It takes a team. Being able to recognize Relative Energy Deficiency in Sport (RED-S) and connect that athlete with the necessary support (not just females) is so important. Mary Cain suffered from this and it likely played a large role in the 5 BSIs she sustained in her time with Nike. Check out the podcast for a of great info on the condition! The pelvis and pelvic floor are under a lot repetitive stress during running and knowing that leakage and pressure are not normal and can be fixed is important. Child birth certainly puts more stress on the area too! These are hard questions and topics to address but knowing what is normal may be the difference between someone continuing to enjoy running and reaching their goals, especially post child birth!
