En 2016, Gwen Jull, déjà connue dans l’univers de la recherche en physiothérapie au niveau de la région cervicale, a publié un article fort intéressant démontrant une corrélation négative entre l’apport des muscles fléchisseurs profonds et superficiels lors de la flexion cranio-cervicale1 . De mon humble avis, et à l’image des résultat préliminaires de mon projet de recherche, je crois que les deux tests d’évaluation de l’endurance des fléchisseurs profonds, soit le test d’en...durance isométrique2 avec la tête non supportée et le CCFT3(cranio-cervical flexion test) sont complémentaires. Le test d’endurance isométrique avec la tête non supportée est rapide à administrer et les compensations faciles à déceler (protraction de la tête, appui trop intense sur la main du thérapeute placée sous la nuque, flexion cervicale active durant le test). Par contre, je considère ce test pertinent comme critère d’inclusion plus que d’exclusion, la raison étant qu’il est possible de passer le test en utilisant de façon dominante les fléchisseurs superficiels. Je commence donc mon évaluation du contrôle moteur par ce test. S’il s’avère positif, je sais qu’il faut adresser le renforcement des fléchisseurs profonds comme démontré dans la capsule vidéo. Par contre, si le patient démontre une endurance supérieure à 20 secondes lors du test, ce qui représente le seuil de passage, et ce sans douleur, je poursuis avec le test de CCFT que je considère plus long à administrer, mais selon moi plus sensible à déceler les faiblesse des fléchisseurs profonds. L’insuffisance vertébrobasilaire - abandon des tests de position selon les recommandations de l’IFOMPT . En 2019, l’IFOMPT (International Federation of Orthopaedic Manipulative Physiotherapists) a révisé et mis à jour son document sur l’ International Framework for Examination of the Cervical Region for potential of Cervical Arterial Dysfunction (CAD) prior to OMT intervention". Dans ce document, plusieurs recommandations ont été proposées dont l’abandon des tests de pos...ition pour les artères vertébrales faute de données probantes. Malgré ces recommandations, et l’arrêt de l’enseignement de ces tests dans la majorité des pays membres de l’IFOMPT, dont le Canada, le débat se continue. He is a board-certified Orthopedic Clinical Specialist from the American Board of Physical Therapy Specialties anda Fellow of the American Academy of Orthopedic Manual Physical Therapists. Prior to his appointment at Daemen College, Dr. Ross served in the U.S. Air Force for 20 years where he practiced with formal credentials to provide direct access physical therapy care, order musculoskeletal imaging, prescribe musculoskeletal medications, make patient dispositions and referrals, and order laboratory tests. He completed his Bachelor's of Science in Physical Therapy from Daemen College, his Master's and Doctorate of Health Science in Physical Therapy from the University of Indianapolis, and a Fellowship in Orthopedic Manual Therapy and Musculoskeletal Primary Care from Kaiser Permanente Medical Center in Vallejo, CA. He has made numerous scientific presentations and has lectured extensively at the entry-level, graduate, and postgraduate levels on medical screening and differential diagnosis in physical therapist practice. Dr. Ross maintains an active practice for community residents, specializing in individuals with chronic pain, and has published over 150 manuscripts and abstracts related to orthopedic physical therapist practice. He served as the Editor for the Musculoskeletal Imaging feature of the Journal of Orthopaedic and Sports Physical Therapy from 2008 to 2016 and is a manuscript reviewer for several medical and rehabilitation journals.
