Kidneys are by far one of the most important organs in the body that’s why there’s 2 of them! Located in the retroperitoneum below the diaphragm, the left ki...dney sits under the the 11th rib descending to L3, while the right one has a lower position, under the 12th rib to the top of L4. These organs are innervated by the renal plexus of the sympathetic system from the levels T10 L2 which enter the kidney with branches of the renal artery, regulating vascular tone and secretion of Renin. The kidneys have multiple essential functions including waste and drug removal, electrolyte balance, blood pressure regulation, production of active Vitamin D needed for calcium absorption and control blood cell production. Besides relationship to structures such as QL, Psoas, ribs and diaphragm, the kidneys also have fascial connections to nerve structures: the subcostal, iliohypogastric and ilioinguinal nerves, and to other viscera: pleura of the lungs, liver, adrenal glands, spleen, pancreas, stomach and descending colon. Kidneys will also refer pain along the lower back, top and sides of the hips and into the lower abdomen and groin. Eerily to the very familiar pain pattern of QL and Psoas. Pair this pain pattern with tension or dysfunction at the T/L junction and an issue such as high blood pressure or history of kidney stones or diabetes and you could be looking at some involvement from kidneys. Treatment to the thoracic lumbar junction, diaphragm, and the kidneys themselves along with their surrounding fascial connections will be greatly effective in helping our patients with issues involving the kidneys such as high blood pressure, diabetes and many others. And Yes! They CAN be treated! Spinal instability can cause low back pain due to the hyper-mobility of the lumbar or cervical segment. Due to many factors such as trauma, arthritis, or degene...rative disease or process, the stability of the spinal segments are compromised and excessive friction and movement result. This excessive friction leads to inflammation and irritation of the nerve roots and the surround structure. Symptoms include back pain associated with movement. Gower’s sign is positive (hand walking on thigh to get to erect position). Low back instability test is positive.
