Nitro...us Oxide started growing in popularity during labour over the last number of years and is offered in many countries, including Canada. Research shows that for maximum pain management, you should start inhaling about 30 to 45 seconds before contraction begins so that the nitrous oxide peaks at the same time as the contraction is peaking. . The benefits of nitrous oxide are that it can create a sense of pleasure and relaxation and reduce anxiety. It helps moms to focus on their breathing and can be used at any stage of labour, including after baby’s birth during the birth of the placenta or perineal repair with minimal side effects to mom or baby. Besides being a lot less invasive than an epidural or injectable opioids, the pain relief with nitrous oxide starts working in about a minute, which is less time than with an epidural. In addition, moms have more control of their own pain relief as they can choose when to put the mask on and when to take it off. It might increase their sense of perceived control which can reduce pain perception. . The cons of nitrous oxide during labor are that it may be less effective than other forms of pain management. Some people don’t like that they have to hold a mask to their face during labour and even feel a sense of claustrophobia from the mask. Differently than you may think, an epidural (or epidural block) does not carry this name due to the numbing medicine that is injected but rather due to the location where it is injected! A catheter is placed in your back and injected in the EPIDURAL SPACE (outermost part of the spinal canal). An epidural normally causes some numbness in the lower part of your body, but the person in labor remains awake and alert. An epidural can done by an anesthesiologist and takes about 10...-15 minutes to be set up and another 10 to 15 minutes to start making effect. Epidural during labor is considered a very effective form of pain relief. Another benefit of epidurals during labor is that it may help moms who are really tired or exhausted from labor, and help them rest and relax, and hopefully even catch up on some sleep -especially for those very long labours. SOUNDS AMAZING, EH? . However, It is important to consider how having an epidural during labor can medicalize your birth. You can think of an epidural as a possible bundle of interventions that goes along with it. Along with the pain management there will likely be continuous blood pressure and oxygen monitoring and continuous fetal and contraction monitoring. You may need extra IV fluids and possibly artificial oxytocin to help augment or speed up your labor, as epidurals can cause labour to slow down. This will lead to reduced mobility during labour! Research also shows that moms who receive an epidural are much more likely to need a forceps or vacuum delivery to help the baby come out at the end of the pushing phase, which in turn increases the chances of severe tears in your perineal area. Also, moms who receive epidurals are more likely to experience low blood pressure. A drop in the mother’s blood pressure can also compromise the oxygen status of the baby, which in turn may increase the risks of needing a cesarean because of fetal distress. In summary, epidural is a very effective form of alleviating the labour pain and discomfort but it comes with possible risks and interventions!
