LUMBAR STENOSIS: THE CLUSTER OF COOK Like mentioned in the last video, lumbar stenosis is a secondary disc-related disorder. Narrowing of the vertebral canal, i...ntervertebal foramen or lateral recess is caused due to hypertrophy of the facet joints and/or the ligamentum flavum.This narrowing is usually increased with lumbar extension and axial loading and can lead to compression of the spinal cord or nerve root, which is another cause for lumbosacral radicular syndrome. The cluster of Cook can be used in the diagnosis of lumbar spinal stenosis amongst patients with back pain with or without leg pain. It cosists of 5 findings that you can obtain during history-taking. If none of those 5 items are positive the chance for your patient to have lumbar stenosis is reduced by a ratio of 0.19. If at least 4 of those items are positive, the chance for your patient to have lumbar stenosis is increased by a ratio of 4.6.
