University of Toronto Researchers Want to Hear About Your Role in Follow-up of People with Diabetes Less Than 45 Responses Still Needed Ongoing monitoring and f...ollow-up is an important part of care provided by pharmacists. However, research has shown that documented follow-up by pharmacists for people with diabetes is low. Now is your opportunity to provide your thoughts about this important issue in a formalized manner that will result in a report and publication to be shared with stakeholders and that will inform the next stage of our research. You will receive an email from Lori MacCallum (Principal Investigator) with a link to complete a short, anonymous survey as part of the study Strategies to encourage pharmacist routine follow-up after initial medication review in people with diabetes in primary health care. We need all of you who practice in community to complete this survey. The survey closes in 7 days and we need to hear from more of you. If you have already participated, we thank you. If you have not yet done so, please take the time to complete the survey. Your participation in research such as this is of utmost value to help move our profession forward. Sincerely, Ontario Pharmacy Evidence Network On behalf of Lori MacCallum BScPhm, PharmD, RPh,CDE Principal Investigator Assistant Professor, Leslie Dan Faculty of Pharmacy Program Director, Knowledge Translation and Optimizing Care Models Program Banting & Best Diabetes Centre, Faculty of Medicine University of Toronto [email protected]
