You may, however, confuse my dog... who loyally sleeps on the floor during our virtual appointment while looking for the person behind the voice!... . Fact Friday . Pelvic radiation can be a component of treatment for many cancers: cervical, endometrial, vulvar, vaginal, bladder, anal, rectal, prostate. . In women, pelvic radiation can cause:... - vaginal dryness - adhesions and fibrosis - vaginal stenosis (narrowing and/or shortening) - tissue changes - gastrointestinal changes - bladder incontinence . Some of these side effects can result in pain and/or difficulty with penetrative intercourse. . A 2014 study showed that 24% of women reported that their radiation treatments had impacted their ability to have a sexual relationship (1). Other studies have reported rates of dyspareunia (pain with sex) to be over 40% (2). . Despite these high numbers, many women are not adequately educated about these side effects. As a result many do not report these symptoms to their doctor and they aren’t referred to trained professionals who can help address these symptoms, if they arise. . Physiotherapists with training in cancer rehabilitation and pelvic health can help these women improve and manage these side effects. . There absolutely IS help out there. . Universally, this has been one heck of a challenging year! . We've learned to adapt and to take things one day at a time. . We've learned to lean on each other in ways we never thought possible. ... . Most importantly, I think many of us have learned to live (even just a little bit more) in the moment and to let go of trying to control the outcome. . Thank you to my friends, family, colleagues and clients for sharing this crazy year with me. It hasn't all been roses, but it's sure been a "bonding experience." . Pre-COVID-19 I had a small bowl of individually wrapped mints in my clinic that clients could help themselves to. . Some people have dry mouths as a side-effect of their cancer treatments and some just like a small treat. It’s a small gesture that I feel is important. . I saw a client today who looks forward to her mint, so I now leave it on the pillow along with a blanket she can use during her treatment.... . Treating the whole person is one of my deep core values, no matter how small the gesture. . Yesterday's announcement of an Ontario-wide shutdown was difficult news for many of us to hear. More uncertainly and difficult times as many of us head into the holidays. . Even though you may not be able to physically be with your loved ones over the holidays, I encourage you to take time to connect with them in the ways that we can. . We need to connect now more than ever...... . With our loved ones... . And with ourselves. . I am thankful that the Ontario government has indicated that physiotherapy and all other regulated health professions can remain OPEN during this lockdown. . Fact Friday . Many breast cancer treatments induce menopause in women, regardless of age. Did you know that this can have a direct negative impact on pelvic health? . Yet women are RARELY asked about their pelvic health.... . Every patient that comes to see me is screened for pelvic health. If there are any "flags" that come up, we have a conversation about ways that we can help. . I recently had this conversation with a new client and it brought her to tears. No one had asked her about this yet she has been suffering in silence. . Just because you've had breast cancer, does not mean you have to suffer and "put up" with the side effects. We cannot control everything, but there are many ways that cancer rehab can help. . Fact Friday . Women being treated with an Aromatase Inhibitor have nearly twice the risk of developing GSM compared to the regular population. (1) . GSM = Genitourinary Syndrome of Menopause.... . GSM is a collection of vaginal and urinary changes associated with the decline in estrogen levels that often occur with menopause. . These changes can cause uncomfortable vulvovaginal dryness/irritation, painful intercourse (dyspareunia) and medical exams, increased urinary tract infections, and urinary symptoms (increased frequency, urgency and incontinence). This can have a negative impact on quality of life. . Did you know that women affected by breast cancer are commonly pushed into menopause, regardless of age, due to their cancer treatments? . Pelvic health is an area of cancer rehabilitation that is largely lacking, but there is help out there. Seeing a pelvic health physiotherapist with specialized knowledge in cancer rehabilitation can be enormously helpful in addressing GSM symptoms. . Interested in learning more? Today is a special day for me. A day full of love, sadness, reflection and gratitude. . 17 years ago today I said my final farewell to my mother, Flora. . As many of you know, my mother was diagnosed with breast cancer at the young age of 40... at a time when "young women didn't get breast cancer." Despite the estimates of 2 years to live, my mother shattered the odds and lived for 14. Though there were many tough days, there were also many healthy and happy ones too.... . Gratitude: To have grown up with such a strong role model. Ironically, I truly feel that cancer gave us the gift of balance as a family after her diagnosis. She is truly the beacon of inspiration behind my career path in cancer rehabilitation and I am forever grateful for this. . Reflection: As I am now in my 40s myself, I continue to reflect and appreciate the lessons she taught me so many years ago. Even though she is no longer with me in the flesh, I know that her impact on my life continues to guide me to this day. . Love and sadness: I think these emotions need no explanation. . Thank you, mom, for all your have taught me. You led by example and showed me how life can continue alongside metastatic breast cancer with both acceptance and grace. . Happy (Canadian) Thanksgiving everyone! . This year has certainly been a different Thanksgiving... no family gatherings, no turkey, roast beef was still frozen when I put it in the oven so major dinner delays (but it amazingly still turned out great!!)... . ... but there is still so much to be thankful for! My hubby and I listed our top 3 what we’re thankful for’s.... . Here’s my top 10 (in no particular order): 1. My husband’s love and support 2. Our new dog Wesley... so grateful he found us. 3. Having a career that I love 4. That we have had a safe home to hunker down in during this pandemic 5. Our awesome neighbours 6. Technology... to keep in touch with my family far away 7. Funny Friday is back this week. The mornings are getting darker and I’m finding it harder to haul myself out of bed at 6am. Earlier this week, while switching into my work clothes (admittedly a little rushed as I was cutting it close to start time), something didn’t feel right.... As I climbed the stairs to my office I realized what it was... my pants were on backwards!!! (The pic doesn’t do it justice.) I chuckled... but then shot a pic to share with you all. Every day at work, my clients meet me with kindness, trust and respect. I am so grateful to love what I do for a living. But what I’ve noticed lately is that stress, anxiety and fear have often joined us in the room. Fear of the unknown. Stress at the schedule and role juggling going on. Anxiety that they (or someone they love) will catch COVID. Anger that they are not getting the cancer tests or treatments they need at the usual speed. As an empath I both see and feel others... emotional strain..... my clients, people in the community, my family, my friends and myself. What if the world was desperately trying to give us a message? What if we tried to take each day one at a time... not plan too far ahead? What if we allowed ourselves to slow down just a little... take a walk at lunch... have your coffee break on the front porch? What if we picked up a new hobby... one that we could do during this pandemic? What if we took a deep breath when we are frustrated.... so we could be more compassionate with ourselves and with others? We are all doing the best we can... this is what helps me cope when someone at the grocery store is rude and snappy. Everyone has a story, even though they may not wear it on their sleeve. With so many happy faces on social media I worry we aren’t always being real. I’m a pretty upbeat and optimistic person but am feeling the emotional weight right now. I’m here to remind you that it’s ok to be wherever you are right now. I wanted to share this peaceful picture of a gorgeous morning I had at the cottage this fall. Picture yourself melting into that rock and feeling the crisp morning air hugging you with warmth and compassion. Hugs to you all. Deep breaths. With so many of my clients working from home at the moment, I’m seeing more people telling me about their sore neck, pain into their shoulder blade or even down the arm.... . Sometimes this brings about natural gut fears that their cancer has returned (or spread). . But many of these problems are being caused by poor home office setups. . Yes, many of us were rushed into creating a work from home setup, and many of you are starting to feel the negative impact of poor setups. . Fact Friday. . Painful sex after cancer is common, but not normal. . Sexual health can be negatively impacted after breast and gynecological cancer treatment. Sadly, these issues (which can have an enormous impact on quality of life) are often never addressed. Many women report high levels of unmet needs when it comes to sexual health information (1-3). ... . There are many simple strategies that can be used to help improve, or eliminate, painful sex (also called dyspareunia). Depending on the cause of the problem, it can sometimes be addressed in just a couple of appointments. . Cancer Rehab Can Help! . As many of you know, I have been saying this a lot this year. My mission for 2020 has been to spread the word about how cancer rehab can help. Sadly, there are still so many people affected by cancer who have no idea that there is help out there. . My #2020believe was to trust that one person can create enough chatter to successfully spread this message. Thankfully, I am not alone in my passion. Today I am joining PINC & STEEL in their movement to ...spread the word so that no one misses out on cancer rehab. They are doing this by creating a Cancer Rehab Awareness Week: Sept. 7 - 11, 2020. . I have posted before about the many ways that cancer rehab can help, but here is a quick summary of some of the common things that we can help you with: - Pain - Fatigue or loss of energy - Weakness - Swelling or lymphedema - Joint range of motion loss - Loss of balance - Pelvic floor dysfunction (incontinence, pain with intercourse, constipation, etc.) - Decreased ability to participate in your usual activities (work or leisure) . Not all physios have the same level of training or experience in cancer rehab and each country is different in how they certify this training. That means it is important (and ok) for you to ask questions when looking for a cancer rehab physio. Ask about their experience and training before choosing your therapist. We are all here to help! .
