This is what we call a catastrophic failure. The in-line adjuster did stretch and twist, but the force was just too much for the steel to handle and it snapped. The adjuster obviously got caught somewhere and hydraulics were definitely involved in this. Fortunately nobody was hurt. This is a polyester/wire rope flip line used in arborist industry. Our flip line is 5/8 diameter braided polyester with a 1/4 Galvanized Aircraft Cable core. Here is a beauty. When the customer heard a loud crack, he stopped everything to find out what happened. One of his crew noticed this and brought to us immediately to be repaired. When you dont use washers to centre your lift point on the shackle this happens. I didnt take a closeup shot of the pin but the threads where the nut fastens were completely stripped away. This shackle was brought to us by a customer. While we were inspecting their rigging our technician noticed this shackle. Rarely do we get to see a shackle with this kind of wear. This hook stretched, bent, and twisted but it did not suffer catastrophic failure(shear). These Italian made hooks are very good. Found this on a wire rope sling when we removed the chain connecting link. As you can see there are 4 broken wires in one strand. This is immediate grounds to be removed from service. This sling was replaced with a new wire rope sling and thimbles were put in the eyes to protect the wires from wear. If you need your rigging inspected, we offer on-site visual inspections of your rigging.
