With the pandemonium about the "Coronavirus" (COVID-19) reaching a peak , clients have been asking if it is safe to exercise? . A 2018 review of the evidence ...surrounding the effects of exercise on the immune system found this : https://www.frontiersin.org/articles//fimmu.2018.00648/full "Contemporary evidence from epidemiological studies shows that leading a physically active lifestyle reduces the incidence of communicable (e.g., bacterial and viral infections) and non-communicable diseases (e.g., cancer), implying that immune competency is enhanced by regular exercise bouts." These findings counter the "prevailing myth that exercise can temporarily suppress immune function." This so called "open window" hypothesis was based on the observation that exercise decreased the immune cell count in the circulating blood for 1-2 hours after exercise. However, further research demonstrated these findings reflected, "a heightened state of immune surveillance and immune regulation driven by a preferential mobilization of cells to peripheral tissues." The authors state, "we emphasize that it is a misconception to label any form of acute exercise as immunosuppressive, and, instead, exercise most likely improves immune competency across the lifespan." . In a recent New York Times article (https://nyti.ms/3cTbnUX), James Turner , the co-author of the above review stated, "there is no or limited reliable evidence for exercise directly increasing the chance of developing any kind of viral infection." He went on to conclude, "so, it is safe to exercise, despite concerns about coronavirus." . So, all our science suggest that it is safe to exercise! One guy comes into the ER with a nail through his foot and no pain. Another guy comes into the ER with a nail through his foot, in total agony... But when they take off his boot, it turns out the nail was actually in between his toes - no damage to his foot at all! Pain is weird, complex, and does not accurately reflect tissue damage. Pain will be different depending on context. How can we use this to enhance our clinical encounters with patients? We discussed this and more at The Grove Health and Wellness grand rounds yesterday!

When someone comes to me with a pain problem, I really try to understand what it's like for them and how it's affecting them. Empathy, perspective changes, and doing joyful activities can change pain experiences - sometimes more so than mobilizing a joint or releasing a tissue.
Great advice for folks with knee osteoarthritis - all the evidence points to staying active! Understanding pain helps too. Did you know that the amount of degeneration on a knee x-ray in no way correlates with the amount of pain someone has?! Talk to your physio about how to get moving and feeling better

I love this new acronym that guides early post-injury recovery, as set out by the high-quality medical journal BJSM. It primes us to expect being active soon after an injury, and points to the importance of mindset and expectations in recovering well. As a physiotherapist, one of my main jobs is to guide my clients through this process, taking into account each individuals specific issues, needs, and goals. A simple but hugely important principle that anyone can put into practice. The flip side of this is that there is SO MUCH your body can do if you build up slowly.
Explain Pain neuroscience education (ie. learning about pain science) helps people with chronic low back pain! 1) improves disability in the short term 2) improves pain scores (improves pain scores slightly, when combined with usual physio)...
