Exciting news... starting on Monday the Buckhorn office will be open 9-3 Monday through Friday! The office is now busy enough to warrant a volunteer to be there all day! Two shifts 9-12, 12-3pm This is great news for the Buckhorn office!

Are you lonely? Need some people to talk to? Worry about being home, alone and isolated? We do friendly visiting calls and reassurance calls 7 days a week ... What’s the difference you say? Reassurance calls are calls that we MUST connect with you daily. If we do not connect within a certain time we call the police to do a wellness check Friendly visit calls are just that... we call you to chat and check in but if we do not connect we leave a message and carry on with more calls! These days with less neighbours to check in and family’s aren’t around as much, these calls have been life saving and mental Heath boosting for so many! Call our office today for more info! During "Community Care Worker Week" of October's Community Support Month #CSM2020, we are featuring some staff members who work behind the scenes on our Collaborative Care Team. This team works with individual clients throughout the City and County of Peterborough. We are starting this series with our Enhanced Care Coordinator Leanne Anderson. Leanne is part of the multidisciplinary team at Peterborough Regional Health Centre. As part of the Home First initiative, Leanne m...eets with patients and caregivers at the bedside to assist with discharge planning. Leanne assesses their need for community support services and collaborates with the team at PRHC to develop individualized, optimized, and timely discharge plans. For over nine years, Leanne has enhanced our partnership with PRHC and the Central East LHIN Home and Community Care staff. Clients and their families tell us that their interactions with Leanne make a difference in their hospital discharge experience.
