Our Adopt a Grandparent program is back! Support a grandparent in Trent Hills, Colborne, Cobourg or Port Hope this holiday season through Community Care North...umberland's Adopt a Grandparent program. Here's how it works: 1) Call 705-653-1411 or email [email protected] to be matched with an anonymous grandparent you'd like to support. 2) You will be provided with a list of recommended/desired items to gift to the grandparent you've been matched with. 3) Call your local Community Care Northumberland office to arrange a time to drop off your gift on or before December 11th. 4) CCN staff will wrap the gifts and safely deliver them to the grandparents just before the Christmas holiday. This year, we kindly ask that you refrain from including homemade meals and baked goods. Non-perishable food items can be included. If you gift a handmade item, such as a mask or mittens, please include instructions on how to wash and care for the item(s). Gift cards to local stores and for CCN's transportation and Meals on Wheels programs are encouraged. If you'd like to adopt a grandparent but don't feel like shopping, contact us about making a donation to this program and allow our staff to do the shopping for you. Gift a grandparent a smile this holiday season through our Adopt a Grandparent program!

From friendly check-in calls, home and outreach visits, delivery of food and other supplies to starting virtual programs and advocating for #digitalequity, the work we do during #COVID19 and recovery is demonstrating there's #PowerInCommunity. In honour of Community Health & Wellbeing Week, we will be highlighting some of our CHCN Community Connectors who continue to go above and beyond to support those in our community and ensure that they receive appropriate services. #CHWW...2020 ---------------------------------------------------------------------------------- Meet Pam C, one of our Social Workers & Pam M, our Community Health Promoter. Together, they offer a Caregiver Support Group on the first and third Friday of every month to provide information, support and follow-up for those navigating new circumstances in their role as caregivers. Previously, the group met in person once per month, but circumstances surrounding the pandemic resulted in Pam and Pam developing new ways to support caregivers given the unique circumstances. They have reformatted their group to a virtual platform to ensure that participants still have the ability to connect with one another, share their experiences and resources, to learn and support, and to grow together. The group now meets twice per month and all caregivers are welcome. Now more than ever, it is important to support caregivers who are experiencing additional challenges including added stress and uncertainty; limited access to care and disrupted service delivery, in addition to countless stressors faced prior to the pandemic. Thank you to our Pams for continuing to offer exceptional support to members of our community!
