Information for Patients and Care Partners: When visiting our Emergency Department, please bring a list of medications you’re currently taking (including injections like insulin and inhalers), your medical history and if applicable, a list of your surgical procedures along with the dates. This will help our Emergency team significantly with the expedition and quality of your care. Most importantly, it contributes to the safe, high quality care we strive to provide daily. If you arrive without your medication list, please call your pharmacy while waiting and they can assist by faxing the list to our Emergency Department at 519.352.6400 (Chatham) or 519.627.9357 (Wallaceburg). #ckha #ckont

As we transition to our new system some things about how we provide care will look different (new devices, etc.) and it may take a little longer as we adjust.
