This includes: * the registration number for the Indigenous child or parent under the Indian Act (if available) * if this is unavailable and the child ordinarily lives on reserve, you can provide information regarding residency on reserve, such as: -confirmation that the child is included in the nominal roll a copy of a recent invoice, bill or notice, showing the child's parent or guardian's name and address, such as a utility bill, or tax notice a signed email or letter from a Band Council member or community health, educational, or social professional confirming that the child ordinarily resides on reserve * the product, service or support needed * how often the product, service or support will be needed * estimated costs (if known) * copies documents related, (such as prescriptions, referrals from a health, social or education professional (if you have them) medical, educational, social assessments identifying the need for the product, service or support *a summary of the child's history and unique needs that should be considered to support the request If you have questions, please call the Jordan's Principle Coordinator in Yellowknife at (867) 873- 4081 ext. 1009.
