POSITION TITLE: Adult Case Manager REPORTS TO: Team Lead, Community Case Management TERM OF EMPLOYMENT: Temporary (8 month contract)... ANTICIPATED START DATE: As soon as possible LOCATION: Calgary, Alberta ** Discovery House is a trusted, dynamic Calgary organization devoted to the safety and well-being of women with children leaving domestic violence. We offer stability, hope and opportunity through long-term residential and individualized community support which result in healthier families and stronger communities. POSITION OVERVIEW: Case Managers provide and coordinate services to assist clients moving from abusive relationships into their own residence in the community. This position is part of the Community Housing Team and collaborates closely with Child and Youth Case Managers and Mental Health Specialists to ensure a family’s practical and emotional needs are being met. Adult Case Managers provide case management services to the families for up to 2 years, sometimes longer, as each family has unique needs. Duties and responsibilities include: needs and safety assessments, coordinate supportive services, help clients develop financial literacy, attend appointments in the community with clients, basic needs referrals, initiate referrals to longer term resources and advocate on behalf of the client. Other duties include: documentation, accurate record keeping, preparing statistical reports and other duties as assigned. This position is Monday to Friday (40 hours) 8-4pm, which also includes a bi- weekly 10-6 pm shift. QUALIFICATIONS AND SKILLS REQUIRED: A bachelor’s degree in a human services field is preferred, but a combination of education and experience will be considered A minimum of a diploma in a Human Services field is required Registration with professional discipline preferred Case management and/or outreach experience required Understanding of crisis management and supportive counseling Understanding of the intersection between domestic violence, poverty and homelessness Understanding of the Housing First model Understanding of Harm Reduction Knowledge of trauma informed practice Ability to work independently and as part of a team Ability to multi-task and work in fast paced environment Creative problem-solving skills Experience working with diverse communities Valid driver’s license and reliable vehicle required. Did you know? Alberta has the third highest rate of domestic violence in the country. Of the 23,247 women and children requesting shelter in Alberta, about 75% are at risk of being murdered.... In Edmonton and Calgary, shelters turned away a total of 13,396 women and children last year. Click the link to donate: www.discoveryhouse.ca/gift-of-hope ** We are a second-stage shelter that provides safe transitional housing, food security, and support to women and their children fleeing domestic violence so they can heal from their trauma, rebuild their lives, achieve self-sufficiency, and break the cycle of family violence. Last year we provided supportive housing to 423 children and 210 women fleeing domestic violence, but had to turn away 611 due to lack of capacity. At any given time, we house 19 families in our shelter and up to 100 families in the community.
