Dear Patients, We would like to clarify the new paperless appointment experience adopted by our office in response to the COVID-19 pandemic. In an effort to reduce physical touchpoints and keep our patients and teammates safe, we are now a paperless office. We are also collecting additional patient intake form to update our records and ensure no new changes in your health history.... This transition will require you to complete two short forms before your appointment. Please note the completion of both forms is mandatory in order for you to receive treatment in our office. 1. COVID Screening Form please complete this form the day before your visit. We will be asking a few simple questions to confirm that you are not experiencing symptoms of COVID-19 and are unlikely to have been exposed to the virus. 2. Patient Intake Form please complete at least two days before your appointment. This form will collect your medical history which could impact your treatment plan. You may have completed this form in the past, and if so, we apologize for the inconvenience. Refreshing this information helps us keep our records up to date and ensures that we know of any changes to your health. We will be unable to treat you until this form is completed. We are here to help you navigate this transition. If you have any trouble accessing these forms or have questions about their content, please give us a call. At Downtown Whitby Dentistry we have been closely adapting to the direction and recommendations from the Public Health authorities during COVID-19 pandemic. We have been thinking about you and hope that you and your families are safe and well. Now we have some Great News!... It gives us an immense pleasure to share that we will be re-opening soon. The provincial government is now allowing health-care providers to gradually resume all services. On May 31st, the RCDSO has released a new set of guidelines for dentists to incorporate within their practices to ensure everyones safety. We are currently working tirelessly to implement the new guidelines including; modifying our office, adding medical grade HEPA filtered air purifiers, extra-oral aerosol evacuators, and sourcing the highest level degree of PPEs. We will contact you once we have a firm date for safely re-opening our practice. Health and safety are our top priority. We look forward to seeing you and your family soon!
