What is advised against? . Medications * The first choice of therapy should be non-pharmacological... * Start with chiropractic, massage, physio, naturopathic care * Advice, education, exercises * If it fails, consider pharmacological (drug) options . Imaging * The use of imaging (MRI, x-ray, CT) should be used sparingly * Medical Doctors and Chiropractors are imaging patients unnecessarily . Surgery * Surgery has a very limited role, if any, in low back pain * These approaches are often used in high-income countries (Canada, US, UK, Australia, etc.), however, there is little evidence of its success . What are the guidelines? . Low back pain should be managed in primary care ... * Back pain should be managed by your chiropractor or medical doctor * Not in an emergency room or ambulance * This helps the patient, saves healthcare dollars and opens up the emergency room for emergencies . Practitioners should provide education and advice * Avoid bed rest * Continue with regular activities * Counselling and cognitive behavioural therapy (CBT) may be beneficial as an adjunct * It is very unlikely that you have a serious pathology . Remain active and stay at work * Many clinicians around the world, incorrectly, advocate rest and absence of work * You will do better with activity and return to work * People are able to get back to work before they are 100%! . Exercise is recommended for chronic low back pain * Chronic is back pain that's been going on for a while (3 weeks - 6 months+) * Find something that you love to do! * Walk, hike, swim, underwater hockey, base-ketball, yoga, pilates, weight training, parkour, snorkelling, etc. . Back Pain Professionals. . Are we Gardeners or Mechanics? . What is a back pain Mechanic?... . There is a breakdown in the system, and we fix it. Back pain is like a car, and there is a mechanical breakdown that can be remedied. Tighten what needs to be tightened, release what needs to be released and swap out parts as necessary. . What is a back pain Gardener? . Provides sunlight, fertilizes, prunes . Meaning: provides education, encourages exercise, nutrition and sleep; balances emotions, reassures the patient they are not broken and provides subtle hands-on therapy (trimming, pruning, releasing) when needed. . Which one is it? . For the longest time, conventional wisdom believed professionals were like Mechanics, however recent research into people as wholistic entities have tilted the spectrum towards Gardeners. . The argument for a Mechanic: . If my back is fine when I rest it but sore when I bend down, it is a mechanical fault in the system. . The argument for the Gardener: . Humans are complex bio-psycho-social beings with feelings, beliefs, conditioning and learned behaviours. Pain is very complex, and blaming it on a biomechanical fault is not true and can be harmful. We are best to shower the patient with sunlight, water and a healthy environment to see them grow. . In my opinion: . After reading the research and through my experiences, I believe we are much more like gardeners than mechanics. . I’m not ready to completely throw biomechanical breakdowns out the window, however I believe a significant shift is needed towards the gardener end of the spectrum. . Low back pain - What are some of the main causes? . Non-specific * In rare cases, there is a discernible cause (fracture, malignancy, infection) * However, most often the diagnosis is "non-specific" - as in health professionals cannot accurately say it is one recognizable factor... . Non-specific * If we can't find a specific cause, can we still "fix it"? * Of course! Rarely is a cause found but people still get back pain relief every day from the right professionals * There are numerous strategies that can be implemented to help improve back pain (more on this in a later post) . However, we have pinpointed some factors. Including: * Physically demanding jobs * Smoking * Mental comorbidities * Obesity (See "Off the Cuff Ep. 4) * Low levels of physical activity . Pain Science and Placebo - Post 2/2 . Do the words that we use affect pain? Heck ya! . Placebo is a substance or treatment which is designed to have no therapeutic value.... . Pain perception is the ability to feel unpleasant sensations. Those with chronic pain typically have a higher pain perception (i.e. they feel more pain). . Researchers from Florida wanted to know if the perception of pain was changed simply by giving people positive or negative instructions. . Methods They split participants into two groups and told them either: -Positive: "Spinal adjustments are an effective way to treat lower back pain and we expect it to reduce your perception of pain"* -Negative: "Spinal adjustments are an ineffective way to treat lower back pain and we expect it to worsen your perception of pain"* -*Paraphrasing. In the study, there was a 3rd neutral group -Participants then performed spinal adjustments (SMT) . Results -Those that were told they would get PAIN RELIEF from spinal adjustments, got pain relief (positive group)* -Those that were told they would get SORENESS from spinal adjustments, got soreness (negative group)* . Take home #1 -Participants got pain relief after spinal adjustments if they believed it was beneficial -With more studies like this, we can make the claim: "If you believe spinal adjustments are good, you may get pain relief. Hamstring Flexibility & Back Pain . Do "tight" hamstrings have anything to do with lower back pain? . What is Hamstring Flexibility?... The ability for your hamstring muscles to lengthen through hip range of motion (typically with our knees extended) . How do we determine Hamstring Flexibility? Three main tests: * Straight Leg Raise (SLR) * Sit and Reach test (+/- chair) * Hip-Knee 90-90 . Japanese researchers examined numerous studies from 1966-2017 & asked the question: "If there are differences in hamstring flexibility and stiffness between individuals with and without lower back pain." . Results "It was impossible to make a definite conclusion in the current literature as to whether individuals with LBP had impaired hamstring flexibility and stiffness." This is science talk for: "No!" . Take-Home Message #1 There is no evidence stating that "tight" or "flexible" hamstrings have anything to do with lower back pain If you have "tight" hamstrings, you may or may not have back pain If you have "flexible" hamstrings, you may or may not have back pain . Exercise to improve pain. . . It is well understood that exercise helps us manage pain. But does that exercise have to be pain-free?... . . Researchers from the UK pondered this question. They wanted to know if painful exercise was better, worse or the same as non-painful exercise to treat pain. . . It turns out that exercise does not have to be pain-free, and painFUL exercise may provide more benefit than pain-free exercise. . . Take-Home Message: Exercise is great for treating your pain and feeling better. Don't be scared to do it, even if it causes a bit of pain. Belief is that if pain with exercise lasts less than 24 hours and stays between 0-5/10 - than you are good to go! . . Research is still in its early stages. Exercise snacks . An exercise snack is breaking up a single session of continuous exercise into several shorter bouts spread throughout the day. . In this study done by McMaster University in Hamilton, Canada; the researchers had the participants:... . Climb 60 steps 3x/throughout the day (spaced out) 3x/week For 6 weeks . The results showed participants had improved cardiovascular fitness and peak oxygen uptake after 6 weeks. . Don't call it a comeback May 31st, I was able to complete something I hadn't done in 3 years - a 1/2 ironman. In 2017, I completed my 1st Half Ironman (70.3) in Muskoka, Ontario, and it was one of the proudest days of my life. 2 weeks later, however, I was travelling to San Fran for a chiro seminar, and there was a 1/2-marathon happening - I had to do it. ... Tissue tolerance is the ability of our body's specific tissue to withstand the effects of load placed upon it. Unless you're the @ironcowboyjames, it is ill-advised to partake in a half marathon 2 weeks after your first 70.3 half ironman to allow sufficient time for overall bodily systems to recover. I suffered a stress fracture in my left medial tibia (the inside of your shin bone) and had to halt all activity for > 6 months. It took almost 2 years to regain pre-injury running resiliencea long journey, both mentally and physically. On May 31st, 2019, I went to watch the 70.3 happening in Victoria because I enjoy the sport and love the energy of all participants. It was that day that I decided I will compete again in this event next year. To prime my body, I then went through 7 months of conditioning at Crossfit Lolo and 5 months of triathlon-specific training. I even hired a coach for 3 months to ensure I was aiming in the right direction. All was set, and then COVID-19 hit - all races were cancelled. This is a #firstworldproblem. It might even be a 1% problem; however, it was still a major bummer. Although I wouldn't be able to compete with other participants and an enthusiastic crowd, I wanted to achieve what I set out to accomplish. So that's what I did . I finished about 12% slower than I did in Muskoka three years prior. However, I did complete all three race goals I had made one year prior: 1. Complete the 70.3 2. Not get injured 3. Stop "Self-adjusting" (@chiroup) .... Back or neck irritation may produce an ongoing urge to "pop it" yourself. Although self-adjusting may produce temporary relief, you should avoid the temptation for several reasons.
