Answer: This is another "it depends" situation (like most tick-borne disease-related questions). . . While a lot of sources quote a relatively arbitrary 24-48hr range, there is evidence that transmission can occur well before the 24-hour mark. .... . Attachment time is just one variable that will affect the likelihood of transmission. And yes, your risk goes up if the tick is attached for 48+ hours. . . Other variables include when the tick's last meal was, the species and/or strain of Borrelia burgdorferi the tick may be carrying, where the bacteria are living in the tick, and if you even remember getting bit by the tick in the first place. . . Other co-infections may also be transmitted before the 24-hour mark. And we don't know for sure how particular co-infections may affect the speed of Borrelia transmission. . . The moral of the story is this: always remove an attached tick ASAP and save it for testing. A patient came in with this creepy crawler earlier this month... the infamous Ixodes/blacklegged tick. . . This guy is most likely an adult Ixodes pacificus, the western variety of the blacklegged tick and a major culprit for tick borne diseases in the Pacific Northwest. .... . So what do you do if you find one? . . 1. Remove the tick ASAP. The longer the tick is attached, the more time it has to transmit whatever bacteria, viruses or parasites its harboring. Remove gently with tweezers or specialized removal tool as close to the skin as possible, pulling straight up and being careful not to squish the tick. . 2. Save the tick. Testing the tick for infections is often easier than testing the human and can give valuable info for preventative treatment. Store in a sealed container with a moist cotton ball. . 3. Follow up with a primary care provider, preferably one who is experienced in treating tick borne illness, for assessment and prophylactic treatment. Bring your tick! If they don't send your tick for testing, find someone who will. . . These are general guidelines and are not meant to be taken as medical advice. Fibromyalgia, chronic fatigue syndrome, and psychiatric illness were the top 3 most common misdiagnoses according to the same study. . . . In addition, the study reported that roughly half of the participants were diagnosed more than 3 years after onset and/or saw at least 5 health professionals before a diagnosis was reached. What do these numbers tell us?... . . Lyme disease can mimic other chronic conditions and vice versa. It is important to gather a thorough history and use accurate testing to discern the difference between Lyme disease and similarly presenting conditions. . . There may be a significant gap between Lyme disease onset and diagnosis for some patients. This can affect disease progression, severity, and long term complications.
