Here's an interesting case I completed yesterday. I performed root canal treatment on tooth 26 a week ago. The canal system was calcified and exceptionally curved. I used my pulpal floor landmarks along with my microscope to search for an MB2. None was found. I obturated, not suspecting an MB2. The fill in the MB canal appeared well centered. The patient continued to have some symptoms beyond the regular post operative period and I decided to do a CBCT scan to ensure ...there was no missed canal anatomy. The CBCT scan indicated the possibility of an MB2 and where this canal could be located in relation to the MB1. Using the CBCT scan, I successfully located and obturated the MB2! Learning point? Think about using 3D imaging to help locate an MB2 canal in a first molar; even with the use of enhanced magnification. See more
