NEW RESEARCH REVIEW POSTED TODAY: Sacroiliac Joint Mobility Tests (MSK Sci Pract 2020) How do you clinically assess a patient for sacroiliac joint pain? Assessment of the SIJ generally includes two categories of clinical tests:... 1. Pain provocation tests: which have shown poor reliability and validity in systematic reviews; and 2. Dysfunction (or mobility) tests: which are designed to assess altered SIJ mobility, although these tests also have not demonstrated satisfactory reliability and validity. Previous reviews have questioned whether SIJ mobility tests are of any value in clinical practice; however, the most recent review was published nearly 20 years ago and any recent studies were therefore not included. Therefore, the objective of this study was to conduct an up-to-date systematic review to verify whether recommendations regarding the use of SIJ mobility tests should be revised regarding clinimetric properties, including reliability, validity and responsiveness, in people with non-specific low back pain, pelvic girdle pain and/or SIJ painenjoy! Which treatment is most effective for patients with Achilles tendinopathy? A living systematic review with network meta-analysis of 29 randomised controlled trials (BJSM - open access) Summary: "In our living network meta-analysis no trials were at low risk of bias and there was large uncertainty in the comparative estimates. For midportion Achilles tendinopathy, wait-and-see is not recommended as all active treatments seemed superior at 3-month follow-up. There seems to be no clinically relevant difference in effectiveness between different active treatments at either 3-month or 12-month follow-up. As exercise therapy is easy to prescribe, can be of low cost and has few harms, clinicians could consider starting treatment with a calf-muscle exercise programme." NEW RESEARCH REVIEW POSTED TODAY: Restless Legs Syndrome Clinical Review (JMPT 2020) Restless legs syndrome (RLS) is a hyperkinetic movement disorder that can lead to insomnia, impacting one’s sleep and overall quality of life. Many of your patients may have this and ask you about it! Symptoms of RLS typically include paresthesia and an urge to move their limbs, generally following a circadian pattern. Currently, the pathophysiology underlying primary RLS is unclear, and... this diagnosis is classified under the category of other specified extrapyramidal and movement disorders. The objective of this scoping review is to synthesize the evidence regarding non-pharmacological approaches used in the management of RLS. Examples of conservative treatments investigated will include exercises, manual therapy, physical therapy devices, and light therapy. To categorize the efficacy of conservative approaches in reducing RLS symptoms, this study sought to identify and summarize the current evidence on diagnostic criteria and relevant outcome measures, informing researchers and guiding clinical practiceenjoy! Great results for chiropractic care of headaches in children! Look for an RRS Review on this one... "Effectiveness of chiropractic manipulation versus sham manipulation for recurrent headaches in children aged 714 years - a randomised clinical trial" (Chiropractic and Manual Therapies - 2021 open access) Conclusions: "Chiropractic spinal manipulation resulted in fewer headaches and higher global perceived effect, with only minor side effects. It did not lower the intensity o...f the headaches. Since the treatment is easily applicable, of low cost and minor side effects, chiropractic spinal manipulation might be considered as a valuable treatment option for children with recurrent headaches." "Comparative Efficacy and Patient- Specific Moderating Factors of Nonsurgical Treatment Strategies for Frozen Shoulder: An Updated Systematic Review and Network Meta-analysis" (AJSM 2020) Conclusion: "Capsular distension is a highly recommended choice for treatment of frozen shoulder, contributing greatly to pain relief and functional improvement; steroid injection is also a prevailing effective intervention. Among new options, extracorporeal shockwave therapy and laser therapy show potential benefits for multiple outcomes. Individualized optimal intervention should be considered, given that treatment effect is moderated by factors including the disease stage, time of assessment, adjunctive therapies, female sex, and diabetes."
