A likely prognosis for scoliosis is spinal surgery. Figure A below shows a large scoliosis curve. Figure B (not the same spine) shows what a scoliosis surgery looks like with metal rods implanted along the spine. This is a very invasive surgery. Although scoliosis surgery does improve the spinal alignment. It happens at the expense of other important factors like mobility and quality of life. There are times when scoliosis surgery may be the only option left. This happens wh...en curves are very large and when curves complicate the organ systems. Preventing scoliosis surgery should be one of the primary goals of care, as many of my patients expressed to me. Scoliosis surgery may prevented with conservative non-surgical methods such as exercises, corrective care, and even bracing. The key is early detection and early intervention. Why early detection? Well, we know that scoliosis can progress rapidly in children, adolescents, and teens because of their growth spurts and because the spine has not reached skeletal maturity yet. But what about adults? A scoliosis in adult can continue to progress at a potential rate of 2 to 3 degrees per year. In adults, the consequence is advance degeneration of the spine, spinal instabilities, pain, and limitations in quality of life. So the earlier the scoliosis is detected in both youngsters and adults, the earlier the conservative care start. Also, the appropriate treatment plan can be given at the right time when the scoliosis is detected early. - - - The xray photos used below are not my cases but instead are stock photos used for illustration purposes. These photos below compare a ScoliBrace versus a traditional scoliosis brace. They are very different. The beige brace is a traditional scoliosis brace. The black brace is a ScoliBrace. Notice the ScoliBrace how it is designed to over correct the posture and spinal displacement. With a traditional brace, if the scoliosis curve measures 44 degrees, it is designed to hold the scoliosis at 44 degrees. The ScoliBrace, in comparison, is a 3D corrective brace. The ScoliBrace i...s designed to reduce the bend, curve, and rotation of the scoliosis in the brace. We can see this in an in-brace x-ray (see photos below). Providing the right brace intervention can make a huge difference for a patient with scoliosis in reducing chronic pain, improving posture and esthetics, stabilizing the scoliosis, preventing progression of the curve, improving quality of life, and avoiding surgery. Scoliosis in Adults... This xrays shows a degenerative scoliosis in an adult. When people think of scoliosis they often think of adolescents or teens. But the fact is, the highest prevalence of scoliosis is in adults. Sometimes its an adolescent idiopathic scoliosis that was not previously identified in the early years. But most commonly, it is a newly developed degenerative scoliosis in adults, called de novo degenerative scoliosis. The prevalence of de novo degenerative sco...liosis in adults aged 60 and up is 36%. In people under the age of 60, the prevalence is 13%. Degenerative scoliosis has serious consequences, including, but not limited to chronic pain, disability, limited mobility/flexibility, and lower quality of life. The most important factor in treating scoliosis is early discovery so that the most appropriate treatment option can be offered and so surgery can be avoided. Treatment may consist of: 1) chiropractic adjustments in conjunction with scoliosis specific exercises, 2) corrective care / traction setups, 3) bracing, or a combination of all three. At our office, we offer chiropractic, exercises, corrective care, and bracing for scoliosis (ScoliBrace). Our new patients are always appreciative when we recommend xrays to evaluate their spine in addition to other spinal, postural, and neurologic assessments during the physical exam. Xrays are very revealing. They will show the alignment of the spine (good or bad), the disc spaces (if they are wide or thin), any arthritis degeneration, spinal curve angles and measurements, any other bony abnormalities and pathologies. When full spine xrays are evaluated from the side, it can ...show the overall saggital balance (how the spine is positioned overall from back to front), and when full spine xrays are evaluated from the front, they can show the overall coronal balance of the spine (how the spine is positioned from side to side). Spinal biomechanics research has shown us what normal alignment should be and what happens when the spine is misaligned both in the side-to-side position (coronal) and also in front-to-back (saggital) position. To keep is simple, the better aligned your spine is from front-to-back and from side-to-side, the healthier your spine is. A healthier spine equates to a healthier and stronger you. But when the spine shifts away from normal alignment, the spine and discs begin to degenerate. In addition, this adds stress to the nerves and spinal cord. Check out the photo below... In Figure A, the normal alignment is shown with the green line. Notice the red lines that the don't match up with the green line. That is where the person's spine is positioned and it's not a normal alignment. Now, in the next 20 plus years, this spine can continue to buckle and degenerate which can cause chronic pain, disabilities, limited mobility, and other health issues. In Figure B, notice where the red arrows are pointing. Look how thin the black spaces (disc spaces) are at the arrows. That is called disc degeneration and multiple levels, L3-L4, L4-L5, L5-S1. Not only do spinal xrays serve as a crucial diagnostic tool in the assessment of the spine, but they can also point the doctor in the direction of what kind of corrective procedures should be prescribed, the likely prognosis of these corrective procedures, how the corrective procedures should be delivered or applied to the patient, and, very importantly, what procedures would be contraindicated based on the xrays findings. When we give this type evaluation and analysis with our patients, they become thoroughly educated on what their problem is, where the problem is, how chiropractic can be utilized to fix it, what to expect with the corrective methods, and how long it would take to see changes or stabilization.

Working from home??? How you set up your desk/table, chair, and computer, makes a big difference. Check out this graphic below. Here are some tips:... 1. Get a good office chair with height control, back tilt control, and seat tilt control (three levers). 2. Set the height of the chair so your knees are 90 degrees (thighs are parallel to the ground). 3. Set the arm rest height so your elbows are 90 degrees and wrists are neutral (no extending too much). This may be a challenge if you are using a desk or table with no keyboard tray. A keyboard tray lower then the level of the desk would make this easier. 4. Set the monitor height so the top part of the monitor is in line with your eyes. This may be a challenge if you are using a laptop at home. If you are using a laptop, I recommend using an external monitor, mouse, and keyboard with your laptop. Then you can raise the laptop on a couple of books so you the top of the screen is in level with your eyes. 5. Use a small pillow in your lower back to keep your back more upright. One of the most common pain complaints I have seen in the last several months is neck pain, upper shoulder tension, and upper back pain because of the stress of sitting in a home work desk. These tips above will make a massive difference. More than pain, bad posture can affect the spine and the discs and the nerves and this can lead to more long term degeneration on the spine and chronic problems. I hope you find these tips useful. Are your working from home? How is your chair and sitting arrangement? Many of my patients have been working from home since March and the biggest problem they face is chair ergonomics. Two tips: 1) Do not use a regular household chair or kitchen chair. Replace it with an actual office chair. It will save your back, your neck, and you will feel overall better. 2) Be sure you get a chair that has these three levers. It will make a world of a difference for you.
