Greetings members of EBNMP Canada, The New Year is here and we hope that you had a restful and festive holiday season. Please take a moment to read all the information contained in this letter. If you have any questions, please email us. This is a reminder that Renewal fees for 2015 are due by March 15, 2015. Your individual renewal invoice for 2015 will be sent by mail. Members that joined in 2014 will notice a pro-rated amount for this year’s renewal.... In order to benefit our members and to ensure the ease of verification for the Insurance Companies, the Good Standing list for 2015 will be updated on the website on April 1, 2015. Updates will ONLY be made at the end of every month (applies to renewals and new members). To assist our office staff in these updates, please ensure that your renewal fees are paid on time. Failure to renew on time will result in member being placed on the suspension list until renewal fees are paid in full with an additional $100 administrative fee. The Suspension listing will be posted on August 1, 2015. Any member that has not renewed by March 15, 2015 will risk being suspended. Updates to suspension list are same as Good Standing updates. For payment by cheque / money order: Please make payable to EBNMP Canada and send to us by mail at 112 - 2400 Midland Ave, Toronto, ON M1S 1P8 along with your 2015 Renewal Form. Please ensure that all requested information is provided to keep your records updated. PLEASE NOTE: Cheques returned by your bank as NSF are subject to an additional fee of $35.00 and an immediate membership suspension until fees are paid in full. For payment by credit card: Visa and Mastercard payments are accepted (additional processing fee of $10 is applicable). Please complete ALL of the requested information on the back of your 2015 Renewal Form and send to the above mailing address along with all requested information / documents. A separate credit card receipt will also be sent out by mail when the transaction is complete. Take note that the DNM, RNP, RAP, RLMP and RNMP designations are trademarked and copy-written titles that are the sole legal property of EBNMP Canada. Only members in Good Standing are entitled to usage. Persons not in Good Standing or not registered with EBNMP Canada, who advertise and hold themselves out to be a DNM, RNP, RAP, RLMP or RNMP are subject to legal action and monetary fines. Failure to renew your membership jeopardizes your investment and you will not be able to claim insurance reimbursement for your services (where applicable). Please make a note on your renewal form if you require any of the following: a letter for city licensing renewal; renewal stickers for 2014; receipt for 2014; Membership Handbook; DNM Hippocratic Oath; Provincial Association Certificate (DNM & RAP only).
