Dr. Shook is once again participating in the Great Cycle Challenge. This is the third year in a row that he will be participating in the challenge. The purpose of the Great Cycle Challenge is to help raise funds to fight childhood cancer. His goal is to raise $500 and ride a total of 200km in the month of August. Hello Everyone! We are all set to reopen on Tuesday May 19th. We have missed you all! Please read the Clinic's protocol for Covid-19, this is what you can expect coming to the clinic for treatment.... FORM Health Clinic COVID-19 Protocol We at FORM Health Clinic wish to ensure all of you that our utmost concern is the health and safety of our patients. We also wish to keep our practitioners healthy so they may continue to serve the community. Clinic members are doing their part by keeping our space clean and sanitized at all times. After each patient: - treatment rooms will be thoroughly disinfected - all linens will be laundered - face cradles, props, door handles, surfaces will be cleaned - debit machine, bathrooms, furnishings will also be disinfected For your part: - We ask if you are experiencing ANY cold or flu like symptoms - fever, cough, difficulty breathing or if you have travelled outside of BC in the past 14 days, PLEASE reschedule your appointment until symptoms subside or you have been tested negative for Covid-19. - For now, all patients are required to wear a mask entering the clinic in common areas. You can bring your own or we can provide you with one at a cost of $2.00. Please let us know in advance if you need a mask. Each practitioner will direct necessity of mask wearing in treatment rooms. - If you have seasonal allergies (sneezing, cough, nasal/eye discharge), a mask must be worn at all times. - Please arrive to your appointment on time, if you are early you are required to please wait in your vehicle until your appointment time. - Patients should enter clinic alone - without being accompanied by family, friends or pets (only registered working dogs), unless parents or guardians are accompanying minors or a person providing aid to those with visual or mobility impairments. - HANDS MUST BE WASHED or SANITIZED IMMEDIATELY UPON ENTERING CLINIC, and again before you leave. - PLEASE limit bringing personal items into the clinic. After washing your hands avoid touching personal items such as Cell phones, books, tablets - DROP-INs are not permitted. - Please respect social distancing while in the clinic in the common areas, please follow floor and chair markings. - Debit or credit are preferred forms of payments. Hi everyone!! Just a quick clinic update. It’s almost time for reopening, we are just waiting on approval from each regulatory body who are working with work safe bc and the provincial health officer, to give us the green light. As such, our various practitioners may have different start dates. We will keep you in the loop as this unfolds in the coming days.... As we may expect, treatments may not look the same in the coming weeks and months, and we will be implementing new clinic policies for both patients and therapists to keep our clinic and everyone in it as safe as possible. Once we receive these procedures we will update you all with our new clinic protocols. We may expect to wear cloth or surgical masks, coming to our appointments alone, physical distancing in common areas, frequent hand washing, limiting items we bring into the clinic, and extensive sanitizing and cleaning to name a few- everything we have become so great at over the last few months!! In the meantime, To answer a few questions: Do I need to call and rebook my appointments that were affected due to the shut down? Don’t worry we will contact all patients who have had their appointments cancelled in the order of cancellation in order to make rebooking fair to everyone. Will my pre booked appointment still be in the schedule for after you reopen? Yes! We have kept all previously booked appointments in our system and we will confirm these as such! For future bookings: After we have rescheduled all of the appointments that were cancelled we will open up online booking to everyone shortly after our opening!! College of Chiropractors of British Columbia As required by the Provincial Health Officer and the College of Chiropractors of BC, during the COVID-19 pandemic, chiropractors can only treat patients in person when providing emergent, urgent and/or essential care. You must schedule an appointment for in-person treatment. Unscheduled, drop-in treatment is not permitted. Before you can schedule in-person treatment, you must be interviewed by Dr. Dyck or Dr. Shook to confirm whether you require emergent, urgent and/or essential care and to assess your risk for COVID-19 infection. Dr. Dyck or Dr. Shook must interview you over the phone or using internet communications. You cannot be interviewed in person.
