It's up to YOU - The BEST treatment/rehab/physical therapy approach should be primarily ACTIVE and serve to achieve one's ACTIVE functional/activity-related... goals, should EMPOWER the person performing it, should promote INDEPENDENCE, and should be served with a heavy dose of evidence informed EDUCATION - In other words: - YOU DO IT - YOU TAKE ACTION - YOU PUT IN THE WORK - ...and you (if needed) work ALONGSIDE a professional (like a physical therapist) who can GUIDE you along that process - If your expectation is to be "fixed" by someone by means of them doing something to you (e.g. massage, joint mobilization, manipulation/adjustments, acupuncture, cupping, etc), you won't achieve the LONG TERM results you want (although they may feel good and help in the SHORT TERM) and you'll waste valuable time and money - If you enjoy these treatments and they help with pain relief, great! - But you still have to put in the work in order to bring about LONG TERM change (whether this includes changing your beliefs, improving physical attributes like mobility, strength, endurance, power, movement quality, etc, modifying your lifestyle and habits, and so on) - This is definitely not easy and takes a lot of willpower, consistency, and mental toughness, but I can assure you that it is the most rewarding and most effective approach! - Questions? What is the reason that you are living with pain? Here are some common reasons that I hear day to day: I thought it would just go away... I thought it was a normal part of ageing I did not know that it could be fixed I don’t have time and it’s not that bad. I only feel it when I move a certain way, so I just avoid that I had it before and it went away I expected it. I was hard on my body when I was younger And what do all of these patients have in common? They can be helped by a physiotherapist and it is easier than they expected. Think of it this way: are the reasons mentioned above a good enough reason to live your life in pain? How can I reduce my pain? Sometimes the answer is simple, and the question is complicated. Pain is complex, and not fully understood. It is not as simple as something is wrong, therefore it hurts. As a physiotherapist, patients are often coming to me with the goal of reducing pain. I believe it is essential for not only the physiotherpaist, but for the patient, to understand what exactly they are trying to accomplish. Therefore, pain education is always at the forefront ...of my treatment. If we understand more about the sensation of pain, then we have a better chance of helping to reduce it. Things that may seem like simple solutions, reduce volume, desensitize the area, or work within the comfort zone, actually have complex theories behind them. Patient profile time: Take a look at the movement of this patients left shoulder blade. This patient is an avid swimmer who developed left shoulder pain a couple of months ago. You can see the difference in the movement of her left shoulder blade compared to her right, called scapular dyskinesia. This was caused by an under recruitment of the rotator cuff muscles and stabilisation muscles under the shoulder blade, and an over recruitments and compensation of the muscles above... the shoulder blade. We used areas of her home to create an exercise program specific to her needs. This included scapular pushups and stabilisation exercises on her kitchen counter and coffee table. She is right on track to pain-free swimming!
