Hi, my name is John Poyner. I'm 54, soon to turn 55 and have lived with a severe form of Muscular Dystrophy, referred to as Becker, since diagnosed at the age o...f 8. As Muscular Dystrophy is a progressive degenerative disease, over time we become much weaker and weaker. Our muscles simply break down and stop working. That's about the best way I can explain my Becker type MD. Personally, I've now had use a power wheelchair for about 20 years or so. No complaints, it is what it is and I well accept that. One can well see and tell you on first meeting, that I'm a very positive, happy going kinda guy. My father my sister and I, moved up to the beautiful Haliburton Highlands in the spring of 1977. I went to both Archie Stouffer's and finished my high school education at here, which I can honestly say were some of the best years of my life. People I presume liked me, or so it seemed, or maybe they were just being kind. It's no matter to me really, it just goes to show you how kind folks are up in Haliburton. Great people, great place to live. I met my wife here as well and we were blessed with a beautiful baby girl, my pride and joy. However our marriage never worked out. My wife left me and her 4 year old daughter... Anyway, long story short...I raised my daughter on my own from then on, no harm, no foul. I did what I had to do as a loving responsible parent. So right from the old Victory Street School, graduating high school, working hard, plus Collage. My daughter also stayed true to this beautiful place we love so much. So in considering my family and I local Hali residents, I would presume is allowed here. Haliburton is a place that sits squarely in the centre of my heart. I do and have always loved it here, just ask anyone who knows me. I'm extremely proud to say Haliburton is my home. I have shopped locally as much as possible... oh I could go on and on but I’m sure you understand my point. However, within the past few years my outlook has immensely changed... What has changed that immensely you ask? The so called services available here for a middle aged, severely disabled man restricted to a wheelchair. I’m personally referring to Paramed in this first post. There will be more posted as this is not just a Paramed issue, it’s also our local LIN (CCAC) problem. It even goes way deeper then that. It’s also a government health services issue, a totally broken system that can’t be bandaged! Let me explain my position... I do need a Hoyer Lift now to get me in and out of bed. My normal visit in the am is 7:30-8:30. When my main PSW is on duty, in the morning, I have no worries but look out if she has any time off! I have even told Paramed repeatedly, for the am visit to get covered when my main PSW is off duty, that I would even accept up to 9:00 am get up time. I really can’t go past that time because my bed time visit is Normally 10:00-11:00 pm.... try being in bed for 9-10 hours or more at my age without needing the bathroom!!! Now here is what’s really has me worried and stressed about! My main bed time PSW is no longer with Paramed and I can’t say as I blame her. This has happened within this past week. Her last night was on Wednesday. Since then here is my coverage... Thursday night, no PSW, Friday night, no PSW, Saturday am covered, Saturday dinner hour, no PSW, Saturday night, no PSW, Sunday am, no PSW. As I write this they also have no coverage for bed time visit again. So now after sleeping in my wheelchair all weekend, i need to get a Dr. appointment because a pressure sore is developing on my bottom! Just frigging what I need, not! No PSW I know and I have had pretty much all Paramed PSW’s, want to work up to 11:00 pm. Well I’m way to young to be going to bed between 9:00-10:00 pm, but to old to stay in bed for 10-11 hours! I’m personally referring to Paramed first. Let me ask these questions For Paramed PSW’s 1. What time is your first morning client and are they honestly a priority? 2. Do you find many clients who use PSW’s as house cleaners and not for personal care? 3. Do you report this to your boss? 4. If allowed would you call our local LIN and report these clients? 5. Do you realize there are probably as many clients waiting to get hours, who could really use them, as those who are selfish enough to use Paramed PSW’s as house cleaners? - For Paramed Coordinators 1. Do you know how to read maps and realize the size of the County and how difficult the roads are here? 2. Can you not tell distance and time between each client based on the above? 3. Why do so many of you mark a client visit that you can’t fill as a Canceled visit even though it wasn’t cancelled by the client? 4. Can you not first call a client and ask them where they could use any hours that aren’t filled, not just put that time in where you feel it best works? For Paramed Clients 1. Do some of you really need a PSW for personal care/light house duties or just a house cleaning service? 2. Do you understand that a PSW is there for the client and only the client, not for everyone living or staying at the clients place of residence? 3. If you do just use PSW’s as a cleaning service and for no personal care, how can you sleep at night knowing your taking hours away from people who could really use them? ----------------------------------------------------------------------------------- **Update... No PSW again last night and once again for tonight's dinner hour and bedtime so far! BTW, I must add that this has been going on way longer then my first post suggests! ----------------------------------------------------------------------------------- Feb.20/20 **Update...As stated on the LHIN website>... Family-Managed Home Care Central East LHIN at 310-2222 (no area code required) or 1-800-263-3877or view our Family Managed Home Care Info sheet for patients and families. http://healthcareathome.ca//Family%20Managed%20Home%20Care Who is eligible for Family-Managed Home Care? A client must be assessed and meet the eligibility criteria for traditional home care, and must have a plan of service developed by the LHIN to be considered for Family-Managed Home Care. A client must belong to one of the four client groups: 1. Children with complex medical needs 2. Adults with acquired brain injuries 3. Eligible home-schooled children 4. Clients in extraordinary circumstances (at the discretion of the LHIN) if the client has a substitute decision-maker, that dedicated person will also be assessed by the LHIN to determine whether they meet the eligibility requirements to take on the required responsibilities under the program, including managing and coordinating the client’s care, and managing and reporting on funding. The client or substitute decisionmaker must also agree to enter into a legal agreement with the LHIN. Who is a client in extraordinary circumstances? In addition to meeting the general eligibility criteria, a client in extraordinary circumstances is someone with relatively stable care needs, but also experiencing extraordinary circumstances as determined by the LHIN upon considering a number of factors, including: Nature, frequency and intensity of care required Unique scheduling issues Language or communication barriers Cultural considerations Rural or remote location of care Lack of available LHIN-contracted service providers. Can a client have a mix of services through traditional care and Family-Managed Home Care? In some extraordinary circumstances, the LHIN may agree to offer a portion of services through the FamilyManaged Home Care program and the remainder through traditional home care. If the LHIN is providing a portion of the same or any service through familymanaged and traditional home care, the total amount of the service will not exceed the amount set out in the client’s plan of service.
