Do I use silicone cups for Cesarean scars? Yes - but I recommend waiting until the scar is VERY well healed (about 6-12 months). Why? I've seen serous fluid be pulled up through hair follicles within the scar before which I find disrupts healing. When I use cups I watch very carefully to make sure the scar can tolerate the amount of suction I'm using. The benefit is that the cups do give a lifting of the scar, compared to normal scar massage that works the scar along the XY axis.

Can you look at your cesarean scar in the mirror? I find a strong association with those who answer "no" and scar pain. Why? I'm not 100% sure. Trauma often plays a role. Anger. Disappointment over a cesarean. Grief over not having a vaginal delivery. Changes in the central nervous system (this is a major abdominal surgery afterall). Postoperative infection. Fear. Probably many different reasons. Sometimes therapy starts by just learning to look at your scar.
Scar itch?! Silicone scar tape is a useful tool for cesarean (and other) scars. It's most beneficial to scars that are raised, pink and itchy. I use this product a lot in the clinic.

There has been quite an increase in knowledge about perineal massage over the last number of years. Here is my recently written article in the @womenshealthdivision newsletter. This newsletter is a great source of info and I encourage all interested physio to become a member of the WHD.
I recently had someone reach out to me, who told me that this post saved them. They were having chronic abdominal pain and it was leading to suicidal thoughts. This post spurred them to have further investigation and treatment into their abdominal nerves. Sometimes we forget about the abdominal and thoracic nerves. They aren't covered extensively in physio school. They can also ve quite tricky, given that there is a lot of cross over in their distribution. But in pelvic floo...r physiotherapy we deal with a lot of patient who have abdomino-pelvic surgeries and nerve entrapment or irritation can certainly happen. Know your distributions, know your myotomes, know your dermatomes. And if you don't know them, that's ok. Send off for a second opinion with someone who does. We can't know it all (don't ask me about wrists, ok?).
