File Request Form: Often decisions are made with out the medical or important information to ensure a proper decision is made by WSIB. You can ensure that WSIB has the information needed by requesting a copy of your file. In your file you can see conversations with WSIB, forms, medical documents, return to work documents, appeal etc.... Once you have time to review your file, you can then ensure the information is there that is missing or communicate a clarification if you see something that is not right. It allows you to ask the right questions. It's your right to get your file and if you have an active claim request it often to keep your copy up to date. You can find the file request form on the WSIB website under forms. WSIB is moving to online files so you may see this as well. Currently not files all are available online.
