TEEN WELLNESS WORKSHOP! Mon Feb 24 6:30 - 8:00pm Aaniin Community Centre *** FREE *** High school age youth can get information from many sources, not all of them credible. This 75 min presentation is for teens, and aims to help them learn strategies to keep themselves healthy both physically and emotionally. The Markham Family Health Team's clinical team composed of a social worker, pharmacist, registered nurse, nurse practitioner, dietitian and occupational therapist will share best practices with the group, and engage teens into thinking about their overall physical and mental health. Fri Mar 9th is the start date for the latest 6-week Chronic Disease Self Management Program! Space is limited. See below: If you have a history of a chronic health issue(s) that has impacted your physical and mental health, this workshop can help to enhance your care. The Stanford University’s evidence-based Chronic Disease Self-Management Program is a 6 week workshop designed to teach self-management skills in a group setting to better manage the common symptoms associated with having a chronic health issue. The targeted symptoms include poor sleep, pain, stress/anxiety, depression, difficult emotions, fatigue, shortness of breath, and physical limitations. On Oct 26, Dr. Allan Grill and Lisa Ruddy RN were honoured to present the Markham FHT Transitions Program at the 2017 AFHTO conference. This program assists patients to transition from hospital to home, and was nominated for a Bright Lights Award for its strong partnership with Markham Stouffville Hospital.
