THYROID HEALTH Your thyroid requires many different nutrients to function optimally Iodine + proteins are needed as the backbone of thyroid hormones. (But too much iodine can be harmful)... Iron is needed to make the enzymes responsible for thyroid hormone production. Selenium + zinc are required for proper conversion of T4 to T3. Selenium is also needed to balance iodine levels. Vitamin D deficiency has been linked to an increased risk of developing hypothyroidism + Hashimoto's Many of these nutrients are deficient in a standard North American diet. I talk to my patients a lot about hormonal imbalances There are a few key hormones I'm looking at when someone presents to my clinic with signs of a hormone imbalance: Estrogen Progesterone... LH FSH Testosterone DHEAS Cortisol Insulin (fasted) Thyroid Hormones (TSH, free T4, free T3) These can all be tested with some simple blood work. It's important when having bloodwork done to 1 Test it at the correct time, and 2 Interpret it properly Many times, women come in having had their estrogen and progesterone levels checked at the WRONG time of their cycle. This leads to incorrect diagnoses if not interpreted properly. It also means the information is potentially useless. For example, your progesterone is MEANT to be low in the first half of your cycle - this doesn't mean you have a progesterone deficiency It is very rare that your hormone levels will actually come back flagged on blood work. This is because the reference ranges are set to look for a frank deficiency, or a hormone-producing tumour. They don't take in to account what may be normal for YOU at the time you tested. Work with someone who understands the intricacies of hormones and helps you get the PROPER testing done. Thyroid dysfunction goes underdiagnosed when we are only testing TSH If you experience a number of the symptoms in either category, our investigation needs to be more thorough. This includes testing TSH, free T4, free T3, TPO, anti-thyroglobulin, and sometimes thyroid receptor antibodies A common time for thyroid dysfunction to present itself is after giving birth. This is known as postpartum thyroiditis, and can contribute to:... Postpartum depression + anxiety Difficulty breastfeeding Insomnia Fatigue Is being a new mom tiring? Yes! Is it stressful? Yes! Will there be sleepless nights? Absolutely! But your concerns should not just be brushed off as "being a new mom" Women who develop postpartum thyroiditis have a 25-30% risk of hypothyroidism in the future. This means these women require more frequent screening. TSH is a brain hormone, not a thyroid hormone A patient came to me recently after struggling to get pregnant for over 6 months. She had already had 2 miscarriages, but her doctor didn't want to refer her to a fertility clinic until she had a third (whyyyy is this the standard?!) She came to me wanting to investigate a little further before trying to conceive again. Every test she had through her doctor "came back normal" (story of my career), but we got a copy just to ...be sure The first thing that stood out to me was her TSH was 3.54 uIU/mL. Yes, this falls within the normal reference range, but we know that a TSH less than 2.5 uIU/mL results in a higher chance of conceiving We ran a full thyroid panel (she told me it's the best $70 she's ever spent) and found low-normal levels of free T4 and free T3, and a high TPO The presence of high TPO (thyroperoxidase antibody) increases the risk of recurrent miscarriage. Up to date research shows that medicating women with a high TPO with levothyroxine improves their chance of pregnancy and live birth Two months after being medicated with levothyroxine, she got pregnant again. She's going to be welcoming her baby girl into the world in just a few weeks If you're not getting answers, find someone who will help you get there. If you're looking to get pregnant in the next year, we can do screening tests to make sure we reduce the risk of concerns like this in the future. "I've been on the pill since I was 14 because I had [irregular cycles, heavy periods, painful cramps, bad acne]" Can the pill help with these symptoms? Sure! But women are put on the pill at a young age for these symptoms (without a work-up), kept on it for 10+ years, go off it in their 20s/30s in hopes of getting pregnant, and then find out there is something going on with their hormones.... If your periods were irregular, were you ever assessed for PCOS? The pill is going to make them seem regular, so you may not know this until you come off it. If they were painful, were you ever assessed for endometriosis? The pill can make periods less painful, but won't necessarily stop the inflammation that is occurring from endometriosis. If they were heavy, were you ever assessed for fibroids/polyps, thyroid dysfunction, clotting disorders, or PCOS? The pill can significantly lighten your flow, but once again does not address the real CAUSE. If you had cystic acne, were you ever assessed for PCOS? The pill can decrease testosterone levels while you were on it, improving acne. But once you go off the pill, PCOS-related acne is likely to come back If the birth control pill is your best option at the time for controlling your symptoms, that's fine. But we are doing young women a huge disservice by putting them on the pill without exploring WHY their period is dysfunctional Moms: This PSA is for you. It's like learning to ride a bike Remember how hard it was to get the hang of riding a two-wheeler? We started on tricycles, then moved to training wheels, then had someone run behind us holding on to the seat of our two-wheeler. We probably fell a few (okay, many) times. There were tears, scraped knees, moments you just wanted to give up This is pretty much the story of every health journey... We can't be expected to go from not riding a bike at all to riding a two-wheeler over night You are going to need a lot of support initially. Sometimes you'll scrape your knee, but we'll get you back on that bike. Sometimes you'll need a few extra tips to learn to stay on your bike. You'll probably need someone to hold the back of your seat. I find so many people are patient with themselves when they are learning a new skill, but expect that they will be able to make health changes and see results immediately. When they can't make the changes or don't see results quickly, I never see them again. These people are at the tricycle stage still, but are trying to ride a two-wheeler without relying on support along the way Be patient with yourself, ask for help. Your period does not have to suck Normalizing all these shitty symptoms means that women may be missing out on getting an important diagnosis. Periods should NOT be a 10/10 on the pain scale. They shouldn't make you vomit, or miss out on social events/work/school because you're in so much pain. If you're told this is normal without investigating, we might be missing something like endometriosis... If your periods are so heavy you bleed through everything, we might be missing a clotting disorder, anovulatory cycles, inflammation, or thyroid dysfunction if you are simply put on the pill or have an ablation to stop the bleeding If young women are put on the pill to "regulate their cycles" before their body even has a chance to actually figure it out, we might miss out on diagnosing PCOS until they are older and want to get pregnant If severe moodiness is laughed off and seen as a woman just "PMSing", we might miss out on diagnosing PMDD Do we wonder why women go on suffering for years? Their concerns are often dismissed or masked, and they are told this is NORMAL. Fatigue is one of the most common things I see women for. Unfortunately it is such a non-specific symptom, so often we need to do some exploring. If you tell me "everything came back normal", but all the doctor has checked is a Complete Blood Count, your cholesterol, and TSH, we're missing a heck of a lot. Some of the most common causes I see:... Low Iron - Yes, even if your ferritin falls within the reference range and you're not anemic, your iron can still be low Low Vitamin B12 - Once again, we're looking for an optimal level here, not just "within the reference range" Low Vitamin D Levels - It's always amazing to see how many people are still deficient even when they are supplementing with the typical 1000IU daily (or sometimes higher). GET YOUR LEVELS TESTED PEOPLE!! Low Thyroid Function - Ruling this in/out requires MORE than just your TSH level Low Protein Intake/High Carb Intake - Not getting enough of the slower burning macros while getting too many quick-acting macros can lead to some crashes throughout the day Burn-out/HPA Axis Dysfunction - High stress, saying "yes" to too many things, not taking breaks, not nourishing yourself... Life is short . . This weekend I lost one of my biggest supporters. Words can’t describe how much he will be missed by everyone who knew him, but we were lucky enough to be left with some important life lessons: .... Use your aeroplan miles whenever you can - don’t let them go to waste. Go on trips, share them with others, but also take trips just to get more points . Make a career out of something you’re passionate about and genuinely love doing. Don’t waste your days doing something you hate . Have that Kool-Aid Jammer, piece of cheesecake, or extra cream in your coffee . Support your friends and loved ones - show them how proud you are, even if liking their Instagram posts about periods makes you uncomfortable . Never stop being a kid. Play games, even if you lose - it’s the fun that counts . Be selfless, give big hugs, sing along to your favourite songs, and spread laughter wherever you go (this might include taking a few jabs at your friends and loved ones) . Why it’s important to always get a copy of your labs . . The number of times I hear Everything came back normal, just to find that everything isn’t normal is... well, way too many times .... A patient came in with concerns of low energy, heavy periods, and hair loss . Her doctor ran some blood work earlier in the year and told her everything came back normal. And sure, everything fell within the normal reference range. But here’s what we found when I got a copy: . 1 Ferritin was at a 9 - I like to see this number closer to 60-80, especially for supporting hair growth and energy . 2 Vitamin D was at 76 - I like to see this closer to 120 . 3 TSH was at 3.50 - Studies show women tend to feel best when this level is between 1.0-2.5. We also ended up running a full thyroid panel to find that her free T4 and free T3 were both on the very low end of normal . . After 3 months of a high quality iron supplement (she wasn’t able to get enough from her diet), well-absorbed vitamin D drops (sorry Canada, but we need more sun), a prescription for desiccated thyroid, and specific dietary changes, her levels are starting to come up to where I want to see them . But more importantly, her energy has gone up to a 7-8/10 (compared to a previous 3-4/10), her hair is falling out a lot less, and she is noticing new hair growth. Her body is now that much closer to supporting a healthy pregnancy - one of her goals in the next year . If we don’t correct underlying deficiencies, it doesn’t matter how much sleep you get, how active you are, how much meditation you do, you’re still not going to feel great . What if I told you to take 1-2 servings of a supplement each night that tastes delicious, will help you de-stress and feel calm, and would probably help you fall asleep. It’s a no-brainer, right? . Why is it so hard to believe that food, lifestyle, herbs, and nutrients can have a positive impact on your health? Why do we need randomized controlled trials to tell us that eating more vegetables and exercising can benefit our health? Yet not a single randomized controlled trial needs to be done on fast food, additives, chemicals, pesticides, food dyes, artificial sweeteners, etc. for people to accept them in their every day diet. How do you support your child's immune system now that they're returning to school? I've had this question from many of my momma patients lately While it may differ for everyone depending on their health history, there are a few key things you can be doing 1 Eat the rainbow - Even if your child is picky, find ONE fruit/vegetable from each colour group (red, orange/yellow, green, blue/violet) and include it daily. Smoothies are a great way to do this... 2 Prioritize a minimum of 8 hours of sleep - If they are having trouble sleeping, reach out or seek support from a sleep consultant 3 Movement daily - Continue to go for family walks, bike rides, encourage them to get outside and PLAY 4 Vitamin D - Enjoy the sun while it still lasts, but also talk to your healthcare provider about a vitamin D supplement. Low vitamin D status is linked to worse outcomes from *certain* viruses 5 Keep sugar intake to a minimum - Added sugar (not from whole fruits) has been shown to depress immune function. I'm all about prevention and optimization Before starting a family, I recommend having a few key tests run to make sure levels are optimal for supporting a healthy pregnancy and reducing miscarriage risks We know that having a low vitamin D status increases the risk of miscarriage, and that improving and optimizing levels improves the chances of a successful pregnancy (psst: this often requires more than the 600-800IU typically recommended for supplementation)... We know that having a high TSH (above 2.5uIU/mL) decreases the chances of getting pregnant We know that having a high TPO (thyroid antibody) even without a change to thyroid hormones increases the risk of infertility We know that having a low progesterone level increases the risk of miscarriage These are all WELL documented findings. So why is it not standard to test these in women who are trying to get pregnant? Why do we not test these until a woman has had at least 3 miscarriages or has been trying to get pregnant, unsuccessfully, for a year? Let's change the standard of preconception care. Movement is Medicine . . I never used to prioritize working out. I never enjoyed it because I thought it meant spending 30-40 minutes doing cardio (I’m really not a fan of running ). Plus I didn’t need to lose weight, so why bother? It just felt like a chore .... We’ve been taught to associate exercising with losing weight. But it’s really NOT a great weight loss strategy. If this is your only goal with exercising, you’re likely going to be disappointed and it will also feel like a chore to you . I found exercises I loved doing, I found someone to push me through my workouts (and now I’m the one pushing him to work out most of the time, isn’t that right @dr.andrewhubbard.nd), and I learned to prioritize MOVEMENTh . Don’t love running either? Go for a walk or a bike ride. Put on your favourite podcast (like @myfavoritemurder - anyone else?), audiobook, music, and just move. Walk around your house if you really need to . Don’t know how to get started? Watch some YouTube videos or join a Zoom workout (@dynamicbodiesfitstudio has some awesome Zoom classes) . Need some extra group support and accountability? If you’re in Halton Hills/Milton, @farmgrrlfitness has some incredible outdoor group classes . Don’t have the time? That’s what I thought too. But all you need is 20 minutes - workouts don’t need to be upwards of an hour at the gym . Positive things that I’ve taken away from the past 5 months: A solid exercise routine - I’ve never really prioritized exercise because I didn’t have time. But when you find something you enjoy doing, it doesn’t feel like a chore (and turns out I do have the time, even when back at work!) Outdoor time - Spending more time outside hiking, working out, going for a walk, or just relaxing has really helped recharge me. And I’ve been so grateful for the hot weather... A better relationship with @dr.andrewhubbard.nd - We actually grew much closer and didn’t get sick of each other, so I call that a huge relationship win! Better work boundaries - Doing virtual visits from home means I really could see patients at all hours. But respecting myself and my time means that I set my hours and stick to them. I’m a people pleaser, so tend to stretch myself thin to fit people in. Having to be firm so I’m not working all hours of the day has been very freeing The last 5 months has been challenging professionally, personally, physically, mentally. But looking back, it’s almost like it needed to happen in order to help me slow down and re-evaluate my priorities. Does anyone else feel like they gained some valuable lessons or took away something positive from this whole situation? PANCAKE SATURDAY . These babies are gluten-free, dairy-free, nut-free, and grain-free for all my sensitive friends out there . .... INGREDIENTS: 3 Eggs 1/2 cup Coconut Milk 2 Tbsp Water 1 Tbsp Maple Syrup 1 tsp Vanilla Extract 1/2 tsp Baking Soda 1/2 tsp Baking Powder 1/2 cup Coconut Flour 1/2 cup Arrowroot Flour 1/2 tsp Sea Salt . Mix together all the wet ingredients. Sift in the dry ingredients and whisk until smooth! Cook on a griddle until golden-brown. SLATHER in maple syrup and butter (it’s the only way) . . I’ve modified these a few times to get them just right, and I think they are pretty perfect now! Rumour has it you can’t even tell they are gluten-free (this is apparently always the goal) . .
