Late Arrivals: Your appointment time will be decreased by the amount of time actually missed. You will still be charged the full cost of the scheduled time. THE COVID - 19 PRESCREENING MUST STILL BE COMPLETED Missed Appointments: These are appointments that are considered a NO call / NO show. You will be charged the full appointment cost. If you have 3 or more misses in a 12 month period, the therapist reserves the right to discharge you from the office. Cancellation Policy: Cancellations with less than 12 hours notice will be treated as a MISSED APPOINTMENT as outlined above. This does not apply if you have failed the COVID - 19 pre screening, or if you come into contact with someone who is under investigation for COVID - 19 or if you have been directly exposed to COVID - 19. If you miss your appointment because of a COVID - 19 related reason, you will not be allowed to rebook until after at least 14 days of self isolation or proof of a negative COVID -19 test.
