This patient was initially seen in January 2020 with an acute apical abscess in his lower left 1st molar. He was very swollen intraorally and extraorally as well. Tooth was opened for drainage and an incision and drainage was done in the buccal vestibule at the height of the swelling. Endodontic treatment was completely shortly after that, after the tooth settled down. Last case before the weekend. Referring dentist attempted RCT on this upper right 1st bicuspid but was not able to locate either canal. The middle radiograph shows how tooth looked when patient came into our office. Using the operating microscope we were able to locate both canals and treat this tooth. This article was just published in the British Journal of Oral and Maxillofacial Surgery A 55-year-old woman was referred by her dentist to the department of maxillofacial surgery, ASST Civil Hospital, Brescia, Italy. During the same afternoon, she had had sev- eral dental implants placed in the upper right maxilla. During the procedure, one of the implants was displaced into the sinus, after which the an...terior wall of the maxillary sinus was opened to remove it. Unfortunately, the dental practi- tioner was not able to see or remove the implant. The patient was therefore referred to the hospital for orthopantomogra- phy, which showed the implant, but not in an optimal view. A computed tomographic (CT) scan was done and showed that the implant had completely migrated 2 cm into the intra- conal compartment of the right orbit. It was located between the posterior ocular globe, the inferior rectum, and the medial rectum, laterally to, and in contact with, the optic nerve. Superiorly, the apex of the implant was adjacent to the intraorbital vessels. The patient did not experience any visual loss or diplopia. She had limited movement of the eye muscles, and some pain and scotoma during ocular examination. The course of treatment was planned in cooperation with the otolaryngology department, with the goal of removing the implant endoscopically to prevent infection or damage to the optic nerve. Under general anaesthesia with orotracheal intubation, we did an antrostomy, ethmoidectomy, frontal tenotomy, and sphenoidotomy. After endoscopic inspection of the maxillary sinus, the proximal end of the implant was identified and deemed unremovable with a conservative approach. We did an enlarged endoscopic medial maxillectomy with a nasolacrimal duct resection, and removed the medial portion of the orbital floor and the lower portion of the papyrus lamina. This approach made it possible to progressively mobilise the implant and extract it with no intraconal bleeding. The postoperative course was uneventful, and she was discharged after two days.

Surgical/Non-surgical treatment. This patient came to us with an upper 1st molar that was failing in all 3 roots. A CBCT confirmed pathology at the apex of the MB and P roots and a long overextension of gutta percha out the DB apex well into the sinus, with associated mucositis. A case of apical surgery performed on a lower left 1st molar that had an acute apical abscess, with the lesion involving the distal root extending very close to the mandibular canal. Interesting case done today with a little of everything. This lower right 2nd molar has: 1) internal resorption in the coronal 1/3 of the mesial root. 2) a MB and ML canal in the main mesial root AND a second ML canal in a separate root way off to the lingual. 3) lateral canal in apical 1/3 of mesial root 4) sharp right turn at terminus of distal root canal... Last patient today had me fooled. Patient had pulpitis in his upper right 2nd molar. The restoration was shallow but a clear superficial crack was seen in the microscope. I did a pulpectomy and upon opening into the pulp chamber I came across a huge resorptive defect just mesial to the MB/MB2 orifice. Some teeth are best sent to the graveyard. This patient was referred to us for RCT on her upper right 1st molar and was very motivated to keep this tooth. However, clinical examination using a microscope revealed multiple deep cracks present in this tooth, which made for a poor prognosis for successful RCT. After discussing all the options available, the patient elected to have Dr. Gold extract the tooth, and have the socket grafted with #mineross from #biohorizons.
