Then a paper came out in 1993 documenting that the success rate for fusion in the Washington Workers Compensation population was only between 15 to 25 percent. I had been under the impression that it was over 90 percent. A lumbar fusion is a major intervention with a significant short and long-term complication rate. I immediately stopped performing them.

Research will not change our profession unless we use it!

What #physios, osteos and chiros don’t want you to know.... #Research showed that the ability to feel with our hands is extremely LIMITED and IRRELEVANT. Making people believe you are skilled in something that doesn’t require skills makes a lot of $$$.... Being able to critically think and challenge your own bias is the only way to create #CHANGE and #IMPROVEMENT. https://thesports.physio//23/more-manual-therapy-bickering/ Unfortunately, there is still a strong culture within physiotherapy all over the world that the more qualified and skilled in manual therapy you are, the more qualified and skilled physiotherapist you are. This is utter bull shit. Being an ‘elite’ physio has NOTHING to do with how well you can massage or manipulate patients, and this needs to be promoted more.

I like this because it gives us a good history of how chiropractic medicine started: a man believed that he cured deaf by manipulating the spine. Chiropractic medicine has been criticized a lot (and with reasons) by the scientific community because many lack the skills to provide effective science-based treatments. Sadly, in my humble opinion, physiotherapy is facing the same problem. For example, advanced manual therapy classes are the gold standard courses required by many... physiotherapy clinics here in Montreal... Which is why I did my level A: almost everything they taught us is outdated >15years old. A pure waste of 800$. I am still very optimistic and hopeful that one day, all professions dealing with pain using conservative treatments will base themselves on research first rather than their personal beliefs. For my dream to happen I need all of your help: 1) Doctors - don’t refer to anyone. Referring to a bad physio/chiro/osteo is the worst thing you can do to your patients. 2) Physios/chiro/osteo - provide effective treatments backed by science first (ex: exercise). Learn about pain science (it is a must nowadays). Then you can do your voodoo shits. I personally prefer not to. 3) Everyone else: For muskuloskeletal pain, see a physio/chiro first... but choose your therapists! When dealing with your health, do not expect anything less than the best... especially when you pay 70-90$ the session. P.S.: pm me and I can give you a list of 10 trustworthy therapists! https://darryl-cunningham.blogspot.com//08/chiropractic.ht

IS HEALTHCARE 2000&Late???! A lot of people say that they provide the most up to date scientific treatments... but how do you spot bullshit? (1) No background in research.... The purpose of research is to inform actions and minimize personal bias (the personal belief that affects your judgement and choices). Sadly, research itself showed that there is a gap of 17 years between the practice of most healthcare providers and research. Does that shock you? Not understanding research methodologies in healthcare nowadays is like playing lotto: you do whatever treatment you think works and hope for the best. The only difference is that if you played the wrong treatment, the patient can get worse. Yet, people believe in you. In orthopaedic healthcare, most people get better on their own!! So whether I randomly poke your back and give you a joke and amuse you while you heal by yourself or provide you an effective evidence-based treatment, there is a good chance you will get better anyways (and NO ONE wants you to know that! Why? Maybe money). This is the reason why QUALITY OF CARE does not equate to BUSINESS SUCCESS in physiotherapy. I hate business but I am forced to do it because of the system. Thankfully, the research allows me, as a therapist, to have good conscience that my treatments are better than placebo and sell my treatments. They also allow me to understand my limits and identify who would not benefit from my treatments. Nowadays, you will ALWAYS find a research that supports your beliefs. The important question is: how do I know if it’s a good research or a bad one?. For me, the main thing that differentiates a good therapist from a bad one is this ability to critically think and evaluate the quality of a research. So... how do you spot the bad ones? Well, my advice is don’t trust anyone and spot the best ones. You, as a patient, is your job to find yourself the better therapist! Here are some clues to know if they are good: 1) They don’t treat everything 2) They don’t brag about being able to fix any problem in a few sessions 3) They have published researches in good peer-reviewed journals 4) They prescribed exercises 5) They stay humble about what they do not know... an expert always feels more ignorant than the ignorant one 6) Their approach is different than 10 years ago 7) After 3 sessions, if you see no improvements at all, they should STOP the treatments by themselves 8)) You can always contact me to get a second opinion
