Hello To Our St. Alban's Patients! To anyone that is booked with us from February 4th-6th: With our dates just around the corner, we wanted to reach out and let everyone know that we have added our Covid-19 Prescreening/ Consent Form to your Patient Portal, under the Medical History tab. ... To make things easier on the day of your appointment, you have access to your "Patient Portal" which should have been emailed to you shortly after you booked in! Once you log in to your portal, proceed to fill out your Medical History. At the bottom of your Medical History Forms we added the Covid-19 Form. Please ensure to fill these out before you come in for your appointment as it makes things a lot easier on the day of your appointment **Tip** This is easier to complete on a computer. If you have access to one, it may be better to use that rather than a cell phone! If you have any questions or concerns regarding the Patient Portal, please reach out to us for assistance!
