Can your gut bugs sabotage your weight loss efforts? Ask the Doc: I just can't lose weight. A friend says that my problem might be due to the types of bacteria... that live in my gut. That sounds crazy to me, but is it true, and can I do something about it? Answer: Your friend could well be right. Here's why. We've known for a century that bacteria live in our intestines, but we've assumed that they did little to affect our health. We thought that they were just mooching off of us taking advantage of the warmth and nutrients in our gut. In the past decade, however, remarkable breakthroughs have allowed scientists to count and characterize the genes in our gut bacteria. The results have been astonishing. Our gut bacteria have 250 to 800 times more genes than we have human genes. Even more remarkable, these bacterial genes make substances that get into the human bloodstream, affecting our body chemistry. That means it is entirely plausible that the bacteria in our gut could be affecting our health. How could they affect our weight? When we eat food, our gut breaks it down into small pieces. Only the smallest pieces get absorbed into our blood. The rest is eliminated as waste material. In other words, not all of the calories in the food we eat get into our body and increase our weight. The gut bacteria help break down food. Some bacteria are better able to chop food into those smallest pieces that get digested, add calories to our body and thereby tend to increase our weight. Theoretically, if our guts have more of those kinds of bacteria, it should be harder to lose weight. But is there evidence that it really is true? Several studies in animals, and some in humans, say that it is. For example, scientists transferred bacteria from the guts of two strains of mice one that naturally becomes obese and one that naturally stays lean into a third lean strain raised from birth to have no gut bacteria. Gut bacteria transferred from the naturally obese mice made the germ-free mice become fat, but gut bacteria transferred from the naturally lean mice kept them lean. Then scientists took bacteria from the guts of human identical twins, one of whom was obese and one of whom was lean, and transferred those bacteria into the guts of lean, germ-free mice. Make sure your diet always includes enough fiber to promote healthy bowel movements, and be mindful about staying well hydrated. If you need help with your water intake, keep a water bottle by your side at all times as a reminder to drink. Jazz up your water with a slice of lemon, lime, or orange. Also, regular exercise supports good bowel function, so if you do not exercise on a regular basis, this is a good time to begin. Don't try to immediately reach the goal of at least 150 minutes per week of exercise. Begin with short periods of walking. Over time, you can increase your speed and duration of activity. Get your doctor's okay if you have any reservations about exercising or you plan to begin a vigorous routine. There are other simple, everyday habits you can develop to help with relief when a hemorrhoid flare-up arises. For example: Less strain, more gain. Don't delay bowel movements during flareups. Always go when you feel the urge, as putting off bowel movements can cause stool to back up, leading to increased pressure and straining, which aggravates your hemorrhoids. Also, try not to sit on the toilet for long periods; this tends to make hemorrhoids push out and swell up. A way to make going to the bathroom easier and faster is to elevate your feet with a step stool as you sit on the toilet. This changes the position of the rectum in a way that could allow for easier passage of stools. Keep the area clean. After every bowel movement, clean the anal area with a witch hazel pad, a soothing baby wipe, or a cotton cloth soaked in warm water. Be thorough but gentle. Aggressive rubbing and scrubbing, especially with soaps or other skin cleansers, can irritate the skin and make your hemorrhoids worse. Applying non-petroleum jelly or aloe vera gel to the anal area can soothe irritation after a bowel movement. Take a soft seat. Sitting on a cushion rather than a hard surface helps reduce the swelling of existing hemorrhoids and prevents the formation of new ones. Regular physical activity promotes general good health, reduces the risk of developing many diseases, a...nd helps you live a longer and healthier life. For many of us, "exercise" means walking, jogging, treadmill work, or other activities that get the heart pumping. But often overlooked is the value of strength-building exercises. Once you reach your 50s and beyond, strength (or resistance) training is critical to preserving the ability to perform the most ordinary activities of daily living and to maintaining an active and independent lifestyle. The average 30-year-old will lose about a quarter of his or her muscle strength by age 70 and half of it by age 90. Just doing aerobic exercise is not adequate. Unless you are doing strength training, you may become weaker and less functional. What is strength training? Strength training encompasses any of the following: Free weights, such as barbells and dumbbells, Ankle cuffs and vests containing different amounts of weight, Resistance (elastic) bands of varying length and tension that you flex using your arms and legs, Exercises that use your body weight to create resistance against gravity. How much do you need? A beginner's strength-building workout takes as little as 20 minutes, and you won't need to grunt, strain, or sweat like a cartoon bodybuilder, either. The key is developing a well-rounded program, performing the exercises with good form, and being consistent. You will experience noticeable gains in strength within four to eight weeks. Getting started Buying your own equipment is one option. Sets of basic introductory-weight dumbbells cost $50-$100. Health clubs offer the most equipment choices, but of course, you have to pay monthly fees. Books and online videos can help you learn some basic moves and start developing a routine. Many senior centers and adult education programs offer strength training classes, as well. However you start, go slow so you don't injure yourself. Discuss your new exercise plan with your doctor and explain the level of workout you expect to achieve. Mild to moderate muscle soreness between workouts is normal, but back off if it persists more than a few days.
