Friday fit tip! I want a lens for this post RK patient, what do I do?!? Empirical order: Designing lenses from topographies like this one can be difficult! The irregularity and corneal scarring significantly reduces map quality, but there are some things you can do to try and capture more useful maps.... 1. Apply artificial tears to help improve tear film quality 2. Take captures in different gaze positions to get more peripheral corneal information. 3. Lift up on the patient's brow to improve lid interference. Because we'll be designing a custom reverse geometry lens for this patient, really try to capture some information about the peripheral cornea to create the appropriate lens landing. Spending a little extra time upfront getting the best quality captures you can will lead to a more accurate initial lens! Diagnostic reverse geometry GP: Because of the significant irregularity, it is a better option to diagnostically fit this patient with a trial lens. That way we know the exact parameters parameters of the lens and how to adjust them to achieve a better fit. Diagnostic scleral fit: You might look at these topographies and say lets just vault all that irregularity and scarring and fit them in a scleral. This is a good option, but keep in mind that endothelial counts can be reduced in eyes like this so oxygen transmission is important. If this patient was in your chair, regardless of the route you decided to take, we can help!
