Pursuit of Success . Meet Sarah! . She came into the clinic in order to manage a nagging shoulder issue. She has been noting this issue in front rack positions along with performing jerks. ... . It has been a pleasure working with her, as she has a commitment to fitness, along with having such a positive attitude (she made us all laugh quite a bit). . After performing an assessment we were able to agree on a treatment plan to get her back to Olympic Weightlifting pain-free. This program involved mobility + stability exercises, specifically targeted at her scapular muscles. The scapula is the stable base of the upper body, without stability in this area the shoulder is not able to move within its ROM efficiently. . We were able to design a warm-up program for her that she can perform prior to lifting. This program aids in mobility along with cueing muscles prior to performing overhead exercises. Give these exercises a whirl! . IT’S REHAB FRIDAY! . This week we will be covering Motor Vehicle Accidents (MVAs) . MVAs can cause unique injuries as everyone experiences different symptoms. If you have a car of 4 individuals and they get in the same accident, they all often walk away with different symptoms. ... . If you are in an MVA, it is important to take a quick look at everyone in your car and to make sure they aren’t panicking, and to see how they are feeling. Make sure to exchange insurance information with the other party, and contact the authorities if indicated. If you do notice symptoms due to the accident make sure to call your motor vehicle insurance provider, and complete what is called an AB1 form (www.alberta.ca/insurance-forms.aspx). You can see a physiotherapist directly (without a physician referral), but it is still a good idea to touch base with your GP. . Common symptoms can include pain + stiffness in the neck/back, shoulders, hips, headaches, dizziness, trouble focussing, and fear of driving. This is why it is important to see medical professionals in order to work through and resolve these issues. Research indicates that 40-60% of individuals can continue to have MVA symptoms for >6 months, so it is important to get help sooner than later. Healthy movement and exercise can prevent this injury from becoming chronic! . With any treatment we address the neck (whiplash), along with the thoracic spine, and any other areas that are symptomatic. The thoracic spine contributes to movements in the neck so it is important to receive mobility exercises for both the thoracic spine and neck (often treatments are only focussed on the neck). . . Meet Silas . Silas is a client that has been experiencing shoulder pain while Olympic lifting. When lifting he notes pain when performing snatches, especially when in the turnover phase of the snatch. During his assessment we determined the root cause, which was a slight inflammation of his rotator cuff (infraspinatus), along with scapular instability. . ... From here we were able to develop clear goals, which was to get back to Olympic lifting with minimal pain and to prevent re-injury. The exercise program that was developed for him during his session is to be utilized as a warm up program prior to lifting. This program is to cue/activate muscles that are key in scapular stability, so when he gets to Olympic lifting (snatch) his scapular stability + motion is optimal. . Exercise 1 Side-laying external rotation with 2.5 lbs 3x7 Exercise 2 Prone 90/90 ext rot holds 3x until fatigue Exercise 3 Scapular push ups 3x7 Exercise 4 Serratus punches 3x7 . Pursuit of Rehab . Meet Richard! He has been seeing Pursuit for treatment since June. He had an ACL reconstruction with meniscal repair in order to provide stability to his knee. . His pursuit is to return to normal function, along with getting back to the gym and recreational sports. ... . We are very proud of his dedication to his rehabilitation, and commend him for his hard work. His exercises include normalizing gait patterns, mobility training, proprioceptive training, and strength training. The current goal is to help him achieve full pain-free mobility while providing stability to his knee in order to prevent exacerbation and re-injury. . To date he has been noting significant improvements with his gait pattern, overall range of motion, and lower extremity strength. This program is designed to help individuals manage their back pain and everyday function. This program is unique as it is being offered virtually! So that means yo...u can complete this program in the comfort and safety of your own home! . The program offers individuals education about back pain, exercises in order to improve strength, endurance, and movement, along with data entry to provide measurable change. . In the GLA:D back program you will complete a detailed assessment which includes baseline testing, goal setting, and objective physical measurements. We would like to welcome Brooklynn to the Pursuit team! She will be completing her practicum as a kinesiology student at our site.Currently she is finishing off her 4th year in the kinesiology program at the University of Alberta. ... Brooklynn has a strong interest in physical rehabilitation and working with individuals with physical impairments.Her interest in rehabilitation began when she had an injury to her shoulder + elbow, when competing in Europe as a Cheer athlete.She completed a significant amount of physiotherapy and noted improvements in her injury along with overall functioning.After this injury she began playing rugby for her high school team. Outside of school she enjoys playing guitar, traveling, hiking, and spending time at her cottage.During the summer months she has a lot of fun wake surfing as well. Meet Michelle! . Her pursuit was to manage her foot/ankle pain, while staying active and performing the activities she enjoys. . Michelle came to us as a client due to a calcaneal fracture, plantar fasciitis, and multi-joint arthritis. These things combined can often lead to significant pain, but can be managed with a program designed to increase mobility, strength, endurance, and proprioception. She is truly a unique case as she has a number of acute and chronic injuries,... but her attitude towards rehabilitation has been nothing but amazing! . Our first project was to work on pain management along with restoring full mobility. This was done through education + self-management techniques, along with home exercises. She was able to begin fully weight bearing ambulation, while minimizing exacerbation. . We were also able to normalize her ambulation! We utilized cognitive ambulation in order to do this. She was able to slow down her walking speed, allowing her to readjust patterns and improve her stability and step length. The first video shows her ambulation patterns without utilizing cognitive ambulation (you can see some instability, and wider steps), the second video allows her to slow down and have a more efficient ambulation pattern. . The next phase of her program was to increase her strength, endurance, and proprioception/stability. There were numerous exercises do to this, which often is the fun part as these exercises are more challenging. Currently Michelle has been managing her pain with minimal exacerbation, and notes significant increases in lower extremity strength and stability! . Great job Michelle! Its our favourite day of the week! REHAB FRIDAY! . This week Sunny explores the temporomandibular joint (TMJ) and associated dysfunctions. Sunny has been working with TMJ cases for several years and notes common trends with these cases. Common symptoms include pain/discomfort along the jaw, ear, face, and cervical spine. Clicking, popping and locking can be common symptoms as well. . We touched on headaches last week, and TMJ dysfunction can be a cause of secondary he...adaches. Ringing in the ears (tinnitus) is a thing to look out for, because we see this with dysfunctions as well. . The causes of TMJ dysfunction can be from motor vehicle accidents, impact to the head, grinding/clenching teeth, arthritis, or anatomical abnormalities. In other cases it can often be difficult to label a clear cause, but we still identify the structures that are causing symptoms. . When coming in for treatment, you will be provided with a detailed assessment in which we obtain relevant information about your symptoms, along with an objective exam to determine a direction of treatment. During treatment we always provide clients with education, self-management techniques, exercise, manual therapy, and acupuncture when indicated. . Common exercises include controlled opening and self-myofasical release. Controlled opening is a great exercise to correct joint mechanics especially if you note stiffness or clicking/popping. Self-myofascial release can help take care of pain symptoms along with headaches, so give them a try! . Sunny works with a number of TMJ specialists, orthodontists, dentists, and physicians. We get straight to the point with rehab at Pursuit! . What you are seeing in this video is a technique that can be performed by physiotherapists, which is known as dry needling. As physiotherapists our main form of treatment is education, self-management, exercise and manual therapy. But additional adjunctive treatments such as dry needling can help you manage symptoms during rehabilitation. . Dry needling releases trigger points (tight bands in muscle fibres), which lea...d to increases in range of motion, decreases in pain + stiffness, with the ultimate goal of restoring function. Dont worry, this form of treatment is very safe, but if you are afraid of needles we still have lots of other adjunctive treatment approaches under our belts. . Meet Melissa! We couldnt be prouder of her progress and commitment to her physical rehabilitation. We often say she has the most interesting shoulder around! This is due to originally having a humeral neck fracture, which eventually resulted in avascular necrosis of her humeral head (bone death). She was able to receive decompression surgery, which was successful. ... She has been consistent with her rehabilitation since these injuries, but notes stiffness along her shoulder, which leads to reduced shoulder mobility. We have been working hard with her to increase her mobility by providing her with self-management techniques, exercise, manual therapy, and dry needling. She has made quick progress and continues to note less stiffness and pain, and finds improvements in overall daily functioning. She is continuing rehabilitation here at Pursuit, and we could not be happier to work with her. She is an advocate for physiotherapy, and we commend her for this! Headaches and migraines are very common in the population, and we would like to provide you with some information of what types of headaches are out there, along with the importance of rehabilitation. . In the world of pains in the head we have two categories, which are primary headaches and secondary headaches. ... . Primary headaches are not the result of another medical condition, and consist of migraines and tension-type headaches. Secondary headaches can be attributed to trauma, cranial/vascular disorders, infection, or issues with the face, TMJ, eyes, or nose. . Tension headaches are the most common type of headache and can affect all aspects of life. They can become chronic if they last for more than 15 days out of the month for 3 consecutive months. 38% of individuals experience tension headaches in a given year. They often occur at the base of the head, the upper back and neck, above the ears, the jaw, or above the eyes. . Migraines can occur for 4-72 hours and result in severe pain. They are often one sided, and involve symptoms that include throbbing pain, nausea, sensitivity to light, sound, odours. 12% of people experience migraines at one point. . Physiotherapy is shown to provide significant reductions in pain symptoms from tension type headaches along with secondary headaches. Manual Therapy, trigger point therapy, along with exercise is shown to significantly reduce their frequency, duration, and intensity. . What is your pursuit? . Our pursuit is to create programs to help our clients reach their goals. . Meet Erin! She had an ACL reconstruction surgery at the end of June and has been diligent in keeping up with her rehabilitation. She was able to initiate a prehabilitation program with Sunny when she initially injured her knee. The goal with prehab is to improve post surgical outcomes and to return to full function more efficiently. ... . The first video was taken on July 22 (approx. 3 weeks post-op) along with the second video. The purpose of these videos was to show Erin how her gait patterns were with and without the use of crutches. She was also advised on how to decrease crutch use safely. . The third video was taken one week later (July 30th), which we see significant improvements. She is not utilizing crutches, and her gait patterns are normalizing. This is due to her commitment to her program while managing her symptoms. . We will continue to modify her program as she recovers, with the ultimate goal of getting her back to soccer and volleyball. . Happy 1 year of being Pursuit of Motion! Today marks the one-year date of opening up this clinic! Our pursuit has always been to provide client centred care that revolves around the goals of each individual client. We couldnt be happier to exist in this community and we hope to continue strong in the future. ... This year has been a whirlwind of new experiences and challenges. Reflecting on it brings many emotions, but one overshadows the rest. That emotion is pride, which keeps us going each and every day. We are proud to be a part of this clinic and fortunate to work with the clients we work with. The process began with the physical build of the clinic. This was a crazy time in which decisions needed to be made quickly and efficiently. The end result was a finished product that has now become a home to many of us. The most important part of Pursuit of Motion is the team. I (Sunny) consider myself the luckiest person to work with the team that I am a part of. From all the fun we have together (it truly is a lot!), to the challenges we work though together, I couldnt be more proud of the team. You guys are the true heartbeat of the clinic and your support gives me the strength to push on. What is your Pursuit? . This week we are highlighting this guy! His name is Melchor and he was involved in multiple motor vehicle accidents over the past year. He sustained injuries in multiple body areas, with pain and lack of mobility being his main symptoms (along with concussion symptoms). . His Pursuit was to return to his full work duties and regain full mobility with his pain being managed. Currently he is increasing his work hours and will be back to full work duti...es soon! . His program began with mobility exercises and a significant amount of pain management techniques (taking regular breaks, stretching, rolling, icing). We were also able to educate him on his injuries and what to expect with his rehabilitation. We then progressed to exercises that would aid him in increasing his strength and endurance in order to return to full work duties. . Currently we are having a bit of fun with proprioceptive exercises as he enjoys basketball along with playing an interesting game of catch. These types of exercises are great because clients can perform meaningful fun activities while getting closer to their goals. . We are very proud of Melchor and we are excited for him to return to full work duties! He has done a great job with his rehabilitation, and we couldnt be happier to be a part of his journey. Guess what? Its Rehab Friday! . This week we will be covering low back pain. This is a very common issue that affects a large part of the population. The causes can be many, and often it is difficult to associate this injury with a specific activity. But we do know that physical activity is very important to regain function and manage pain symptoms. . Firstly, set you recovery expectations! It is shown that individuals with poor recovery expectations are more at risk f...or poor outcomes. Since recovery expectations are a strong predictor of outcomes it is important to stay positive and celebrate your wins! . Research indicates that physical activity is important in order to return to functional activities along with symptom management. Aerobic activity is shown to decrease pain when compared with other types of treatments. Get on a bike; go for a walk, or even a light jog! It is very important to get moving (within pain tolerance) and to remain active! . Dont forget about mobility training + stretching! With any type of injury we want to encourage pain-free motion. This will also aid in the management of general pain and stiffness. The stretches in the videos consist of the Childs Pose, and the Quadratus Lumborum stretch. Each stretch can be performed 3x30 second holds. . Lastly, try to complete some resistance training. This will help improve your core and low back strength along with motor control. The goal would be to utilize these muscles in daily activities to prevent further injury. Examples would include the good morning exercise and planks. The good morning exercise is a great way to engage the lumbar spine musculature along with the hamstrings and glutes (try 3x7 reps). The other exercise is the infamous plank! This can be performed in multiple ways but remember to try to maintain good posture. Once you cannot maintain the correct posture then take a short break (try 3x until fatigue). . Common tools can include a foam roller or a small ball. . The best way to roll out is to roll into areas of tension/soreness and hold that position until the discomfort dissipates, and then move to the next spot.... .
