Meet Christine Cook, an RN on our ACE unit. Her project on improving the transition home for patients with dementia was chosen from projects across Ontario for an Advanced Clinical Practice Fellowship (ACPF) grant of $15,000 from the Registered Nurses' Association of Ontario to further help her with her work: helping our patients to further receive the best care we can give them, both in hospital and beyond.
