The Evolution of Acne By Dr. Danny Guo Pimples, zits, blackheads, whiteheads, cysts, and pustules are some of the terms that have been used to describe acne. Whatever you want to call it, it is a common skin condition that affects a whopping 85% of young adults. With itch and pain being its predominant symptoms, as well as the potential for cosmetic disruption and scarring, it is no wonder that acne is one of the most common skin conditions that dermatologists see in cl...inic. While it is a common and relatively benign condition, it is important for us to have a clear understanding of how acne behaves so that we can treat it appropriately. This is especially important in a time when both accurate and inaccurate information are readily available to anyone who has access to the internet. So let’s take a deep dive into the evolution of acne! Acne starts from two simple processes: your pores get blocked from sticky skin cells (keratinocytes) and get filled with your natural oils (sebum). When these two happen together, your pores turn into comedones. If the opening stays closed, these become 'closed comedones' AKA whiteheads. If it opens, the contents get exposed to oxygen and turn black, becoming 'open comedones' AKA blackheads. As the pore becomes more filled with sebum, a type of bacteria called cutibacterium acnes grows and causes inflammation, transforming the comedones into inflamed red bumps and pustules. If the acne continues to progress, the comedones can eventually transform into cysts or form scars. Naturally, the best treatment for acne will depend on the type and the stage. For example, treatment of whiteheads and blackheads should target the clogging of pores, while the treatment of red bumps and pustules should target inflammation. Not only will using the wrong form of therapy be ineffective, but it may also cause undue side effects and allow the acne to progress, thus potentially increasing the risk for scarring and other complications. With literally dozens of different treatment options out there, it is important that you visit your family doctor or dermatologist before starting any form of treatment. See more
