Who's ready for the Rocket Doctor / Bridge To Health USA COVID-Corner? This morning's question is for our fearless healthcare workers on the front-lines. **Please note that this is a challenging question to answer, and depending on access to resources what's available may need to look different: ... Question: How bad is the aerosol to the surroundings in non invasive ventilation BIPAP and for CPAP. Answer: Non-invasive ventilation (NIV, including BIPAP and CPAP) have been shown in past outbreaks such as MERS and SARS to be a risk factor in spreading virus and infecting healthcare providers (HCPs). BIPAP is less worrisome than CPAP, which is if possible avoided in COVID patients (although in some areas where ventilators are full, CPAP with innovative masks are being trialed). BIPAP is still more likely than invasive ventilation (endotracheal intubation) to produce large droplets and aerosols. These droplets are mostly confined to within 1-2 meters of the patient, and may be similar to a COVID-19 patient who is coughing or sneezing. If the non-invasive ventilation system has good interface fitting, exhaled air is not widely dispersed. Good mask fit is key to reduce the spreading of aerosol particles, but all healthcare workers entering a patient's room on NIV (including BIPAP) should be in full airborne protective personal equipment (PPE). Numerous medical societies and associations state that any NIV should be avoided in COVID patients, both because of the increased risk to HCPs and because it only delays the gold-standard both for safety of HCPs and patient outcome: intubation (with an inflated cuff). However, there are some recommendations that BIPAP may be trialed for patients who have conditions such as COPD / CHF. Although, if BIPAP is to be attempted all necessary airborne precautions should be taken, including the patient in a negative pressure room. As well, if the patient is deteriorating, there should be no delay to definitive airway management. Please be certain to consult local professional bodies and hospital / public health procedures and guidelines before making any clinical decisions that may impact patient care. Good Morning! The COVID-Response team recently took a look at an important topic these days: Going Outside! Here's the question asked on our website (generalized to the public):... Currently, my family and I are going for walks outside. Sometimes people have been jogging by us / we pass other people. Should we not be going on walks? Here's the answer from our team: As a rule of thumb, it is important to follow the public health guidelines given by your local government offices. In San Francisco, a densely-populated city, for instance, folks are recommended to only go outside to take care of essential tasks, such as acquiring food, toiletries and/or medicines, whereas in other cities that are not as densely populated or do not have shelter in place requirements, the recommendations may be more relaxed. Please first check on your local government office’s public health recommendations and follow their guidelines. Utilize the local department of health website and resources to obtain locally relevant guidance. If there are no strict guidelines about walking outside, taking walks will not only help you get physical exercise, it will also help improve your mental health, which is just as important to cope with this pandemic. The recommendations delivered by sources such as the San Francisco city government is to stay at least six feet away from other walkers/runners to decrease the risk of exposure. I got a message from someone un...der quarantine in Italy who asked if I could make a graphic about how to handle the anxiety and fear of this whole thing and honestly that's hard to do because I'm anxious and afraid too. The CDC and Mentalhealth.org.uk put out some great tips so I shared them in a blog post (link in bio) and made this image. It's okay to be scared/anxious/depressed over what's happening around the world. It's okay to talk about it. It's okay to ramp up the self-care. It's okay to seek extra mental health support.
