To ice or not to ice? There is much conflict regarding head injury treatment in light of recent attention on Concussions. We must ensure that we understand the two types of injuries - head injury vs traumatic brain injury. If a casualty acquires an injury to the face /head that we can visually see trauma, and they complain of pain, a cold ompress should be applied immediately for best results (or at least within the first 24-72 hours). The goal is to reduce swelling: here is ...the pathophysiology... The bodies initial reaction to the application of cold is vasoconstriction, or the tightening of blood vessels. This helps reduce swelling by limiting the amount of blood that can flow to the area. As the cold penetrates the skin and tissue below the skin, it slows down cellular activity. This means that there is less cell division, less cell death, and less production of cellular waste. This also helps control swelling as the bodys natural anti-inflammatory response starts to kick in. The key to successful first aid in a head injury is also REST (so no further activity increasing blow flow), and to watch for signs & symptoms of BRAIN injury (drowsiness, confusion, memory loss, vomit, delayed/unequal pupil reaction, severe headaches, etc). In the case of a major head or brain trauma, there will be no time for a First Aider to apply cold therapy, as the priority of care will focus on ABCs, managing seizures and/or blood, brain matter or exposed skull. So to ice, or not to ice? In the case of a head wound/injury, go ahead and use whatever you can get your hands on that is cold. Ice should never be applied directly to the skin, wrap it first. Then keep an eye out for concussion symptoms, and discontinue play! SAJE/Vital Signs is offering a new 3 hour course on Concussion Management. Congratulations to Chris Riley, one of SAJE/Vital Signs' First Aid Instructors for having a hand in saving this mans life! We have reports that his teaching efforts resulted in a save, following a cardiac arrest when one of his students used CPR, and recalled that they could hear Chris' Instruction when performing the skill. Chris is a fantastic Emergency Response Instructor and Instructor Trainer and anyone he teaches only wants to be retrained by him again. Thank you Chris!

Time and recognition are crucial in the treatment of a Stroke (Cerebral Vascular Accident). The casualty only has about 3-4 hours from symptom onset to get to a Stroke Centre for treatment that may reverse symptoms. Call 911 whenever Stroke is suspected and remember to act: F - facial droop A - arm drift/ weakness S - slurred speach... Heat Alerts across Ontario continue! Certainly odd in September. Remember to keep hydrated and check in on pets and elderly people who may be living alone. The key to First Aid for Heat Stroke and Heat Exhaustion is PREVENTION. In addition to staying hydrated, ensure that you are keeping your body's electrolyte balance with salt and sugar intake as well. Stay out of the heat by frequently taking breaks in cool, air-conditioned areas, and if you like to exercise outdoors, save it for the cooler times of day such as early morning or late afternoon.
