Our lovely patient had unfortunately chipped her upper and lower teeth from biting her nails, so she was looking to restore the chipped edges while also closing the gaps between her upper anteriors. While she watched two episodes of greys anatomy, we managed to restore balance and symmetry to her smile using @bioclearmatrix with a bit of @gcamericainc geanial flowable composite in shade... BW and adding a final layer of @dentsplysirona.canada TPH spectra in shade BW to the facial surfaces to brighten up the colour! W H I T E N I N G & B O N D I N G If you can believe it our back to back posts are actually sisters! This case was actually a long time in the making ... well, just over a year. Our patient had actually struggled with really a painful gum condition while she battled some health issues over the past year - acute necrotizing ulcerative gingivitis it’s a mouthful to say and not always fun to be on the receiving end of it either. So with covi...d and everything making it difficult to have her come in for regularly cleanings, we FINALLY managed to stabilize her tissues before being able to whiten and bond her fractured teeth. So today was the day! B O N D I N G This was such a fun case to do! Originally we were simply planning to bond the two front teeth only to correct the enamel deficiencies that you can see on the facial surface. But once the patient mentioned that she wasn’t happy with the overall colour of her teeth, we whipped out our colour guide to see what we were working with... and it looked like we had several different shades on our hand. When I asked her what kind of regimen she was doing at home, she m...entioned charcoal toothpaste - not a good idea to be using because it can strip away the enamel and actually make the teeth look more yellow and translucent over time, which is what we are seeing here. The rest of her teeth were a pretty nice shade so I figured let’s restore the facial surfaces of the 6 upper anteriors and make everything match, because whitening definitely wouldn’t give us the result we were looking to achieve. Q U I C K F I X Quite a unique case! This patient’s concern was that he had two of his front teeth that seemed to be shorter, but that his smile hadn’t always been this way. Upon clinical examination, we found that his 12 and 11 were in fact ankylosed (fused to the jaw bone) as they made a distinct sound when I tapped on them When we discussed his options, the most convenient treatment would be a direct build up of the teeth using none other than my favourite restorative... material, composite bonding. So with @gcamericainc genial sculpt composite in shade A1, we lengthened the 12 and 11 while correcting the colour and restoring the enamel defects. B O N D E V E R Y T H I N G Quite a unique case we were working with today! Our lovely patient who happens to be a dental hygienist in the making is congenitally missing her two lateral incisors. After having braces, her dentist made her this very interesting albeit effective contraption to replace her missing teeth and fill in the gaps left behind for future implants. The only issue is that the appliance was extremely unstable and moved every time she spoke, not to ment...ion the acrylic teeth that were mildly disproportionate to her central incisors with some discrepancy in the length and shape. So of course, my solution to everything ... bonding. First we trimmed the acrylic teeth to widen the centrals, then reshaped the laterals while adding a bit of length and improving overall symmetry. The final touch was stabilizing the appliance behind the two central incisors to keep the sucker in place! After all was said and done our patient finally felt like she was on solid ground again when it came to her smile! This lovely colleague of mine had gone through not one, not two, but THREE rounds of ortho and unfortunately was still unable to get the result she had hoped for. Her main goal was to create a whiter and broader smile by proclining the teeth while closing the gaps around her peg laterals.
