PARACETAMOL IS NO BETTER THAN PLACEBO FOR ACUTE LOW BACK PAIN. Findings from a Cochrane Collaboration systematic review [1] show that paracetamol does not produ...ce better outcomes than placebo in the management of acute, non-specific low back pain. The systematic review found that there is high quality evidence that paracetamol (4g per day) has no effect on quality of life, function, global impression of recovery and sleep quality at 1, 2, 4, and 12 weeks. There were also no significant differences between paracetamol and placebo for adverse events, patient adherence, or use of rescue medication. A more recent study [2] supported the findings of the Cochrane review, that there is increasing evidence that shows paracetamol is not more effective than placebo in patients. with acute low back pain. Concerning patients with subacute and chronic back pain, the evidence for or against the efficacy of paracetamol versus placebo is lacking and would need more research. However, the authors concluded that a strategy focusing on non-pharmacological management as the first treatment option in low back pain may be equally effective with fewer side effects. Chiropractors undergo extensive training in non-surgical, non-pharmaceutical interventions for the management of spinal pain and disability. Manual therapy, exercise prescription, lifestyle advice and patient education are all part of the biopsychosocial approach to care favoured by evidence-based chiropractors in clinical practice.
