Believe it or not, the ‘before’ photo is actually 2 weeks after anti-wrinkle injections to the crows feet. Did the treatment not work? Why are they still there?... . Anti-wrinkle injections in this inferior part to the lateral canthus (bottom of the crows feet) need to be treated with caution on mature patients. Why? There is often loss of volume from fat pad atrophy, thinning of the skin and skin laxity. If you go too heavy handed with the anti-wrinkle in this area, it can 1. Make the patient look sad, as it relaxes the muscle down in this part of the eye. 2. Give an appearance of what we call shelving. This is when the patient smiles, it creates a ledge where there obicularis oculi has dropped and the other smile muscles are pushing up under it causing the lax skin to bunch up to create a shelf-like appearance. 3. If anti-wrinkle is injected too low and deep, there’s a risk of hitting the zygomaticus major and minor (the other smile muscles from the corners of the mouth up to the cheekbone) resulting in a crooked smile. . It doesn’t matter how much anti-wrinkle you put in this area, if there’s loss of volume (lack of support), it’s never going to work to smooth the lines. The skin needs support under it. Additionally, your skin treatments also come into play in this area. . Don’t be afraid of cheek fillers making you look fake, even if you have good cheeks.
