Another common scenario we come across...worn and shortened incisors. Patients usually ask if they can just do 4 teeth. What is your answer? First you have to diagnose why the edges are shortened. Was there compensatory eruption? If you lengthen the teeth by raising the gums then maybe 4 is ok. If you lengthen the teeth incisally, do you lose canine protected occlusion? Is it impinging on the envelope of function? Do you need to raise the bite? Do you need to prep significant...ly? Do you need ortho? Those are some things to think about. Here, @freeman.caro.lands.ortho @drbvf intruded and retracted the lower incisors to create space for ceramic veneers at the top so teeth can be lengthened without much preparation. No anesthetic needed for this case. Canines and premolars veneers were included to increase canine guidance and change color and arch form. Upper and lower veneers to restore significant wear. Additive posterior onlays to open the bite and to protect exposed dentin. It’s all about space...you either create space orthodontically, prepping teeth, or opening the bite. Between prepping (subtractive) and opening the bite (additive), the latter makes way more sense to me especially in wear cases. More issues with laterals...this is quite common. Congenitally missing laterals that were replaced with canines that were moved there orthodontically. We then end up with large teeth in a place where it should be small. To complicate the matter, there are gaps on either side. Generally we can improve the aesthetics by involving the front 8 teeth. Here’s my reasoning: the natural canines need to be smaller to look like laterals so the gaps will be even bigger. Then the central...s need to widen distally to fill out one gap. The 1st premolar (now in the canine spot) needs to also widen mesially to fill the other gap. The 2nd premolars then also need to widen to close any more spaces and also to be brought out so it is not overshadowed by the 1st premolars. This allows the 1st premolar to only widen mesially whereas if it enlarged on both sides it would look way too big. Obviously if gaps were smaller then less teeth can be involved. Significant wear on one side due to a previous cross bite. Ortho improved the position first and then veneers and posterior onlays were done after to fully restore the length of teeth, improve posterior occlusion and jumpthe cross bite.
