Our hearts go out to everyone effected by this workplace incident. https://www.cbc.ca//worker-crushed-to-death-at-burnaby-con There are so many factors at play when an incident is caused by mechanical failure. (Just to name a few)... Have you completed regulatory maintenance to all machinery and equipment as per manufacturers specs? If yes, was the person conducting the above maintenance adequately trained? If yes, was there documentation of a regular maintenance schedule, and training certifications? Were frequent pre trip / inspections completed? Was a FLHA completed prior to work and with the introduction of new equipment brought onto site through out the day, to ensure all site crews were aware of the hazards? The scariest part of all ...is that incidents can still occur whether or not you did all of the above. We cannot always control or catch a mechanical failure, just like we cannot control others on the roads and their driving habits. Ensuring you are completing your regular inspections, training, and site safety requirements can help us put controls in place to help minimize a hazard turning into a catastrophic event.
