What drives us? Stratim is dedicated to helping your organization be the best version of itself and deliver on the promise you make to the people you serve every day. Our work in healthcare performance became a focus when a series of personal experiences with the healthcare system heightened our drive, focus and resolve. The healthcare system is delivered by many dedicated people who only want to do their best for their friends, families and communities, but people make mista...kes and systems have weaknesses. These weaknesses sometimes mean that people fall through the cracks, get hurt or die unnecessarily. My father, a best friend and two close relatives died or almost died due to failures in the healthcare system. My best friend died due to a misdiagnosis coupled with the failure to submit a test sample on time. By the time this was discovered, cancer had taken hold. My father died as a result of a surgical mistake. When the one lung he had left collapsed in surgery, it was not monitored overnight and collapsed again. These are only two of my first-hand stories. I have heard many more from others. Healthcare is a system under pressure. The pressure is growing with aging baby-boomers, changes in system structures and accountabilities, changes in our environment and mass events like Covid. Social determinants of health mean that the web of cause-and-effect extends beyond healthcare into broader services which can contribute to better health and quality of life. Balanced Scorecard The Stratim Balanced Scorecard can be integrated quickly and easily from a wide variety of sources to provide a holistic view of organizational performance and analytics. It comes pre-loaded with a set of performance measures as starting point examples. The performance measures and outcomes are aligned with the Quadruple Aims. The model is highly customizable so that you can add, remove or change virtually any aspect to suit your needs. Stratim wants your input! There are almost 20 HPCO agencies who have joined the Stratim #HPCOCollaborative with more expected soon. We are organizing a series of web conferences to discuss what the members would view as organizational performance priorities. One example is the development of a residential reporting model that takes data from your internal system to provide the data which report on a quarterly basisavoiding the pain of manual tabulation. Other examples migh...t be tracking medication incidents; volunteers; staff turnover; fundraising; whatever We would like to know what other requirements that you have. What data is important to you: for internal reporting, board reporting, external reporting, quality improvement, or whatever else you can conceive of? This will help us facilitate collaboration and build on models which we already have to meet your needs. Several dates and times will be provided shortly, with the first meeting next week. We know that not everyone will attend each meeting. A summary of the results will be provided to attendees.
